Microvascular decompression: Diversified of imaging uses, advantages of treating trigeminal neuralgia and improvement after the application of endoscopic technology

Abstract: Classical trigeminal neuralgia (CTN) is a unilateral and severe facial pain disease, which seriously affects the patient's quality of life. Microvascular decompression (MVD) is currently the most effective surgical method, and it is the only treatment for the etiology of CTN. Imaging for MVD has been increasingly used, and the advantages and disadvantages of endoscopy-assisted vascular decompression surgery have been controversially debated. In this review, we aimed to discuss the advantages of MVD in the trea… Show more

“…El tratamiento es variado, incluye un tratamiento farmacológico, quirúrgico, radiocirugía y gamma radiocirugía, siendo el más común el farmacológico y la descompresión microvascular del nervio pág. 931 trigémino, descrito inicialmente por Dandy y promocionado por Jannetta, sin embargo, no garantiza ser una cura definitiva, por las posibles recurrencias de los síntomas (3,16).…”

“…The selection of this technique depends on the accurate finding of neurovascular compression by preoperative imaging. In addition, magnetic resonance imaging plays a diverse role in MVD, not only in identifying the responsible vessels but also in determining prognosis and as a tool for scientific research [8] . Accurate diagnosis is crucial because neuroimaging interpretation and clinical management differ among the various forms of facial pain.…”

“…CTN significantly impacts the quality of life and daily functioning of those affected, highlighting the need for a deeper comprehension of its underlying pathological mechanisms, however, its precise pathological mechanisms remain elusive (4,5). The current understanding suggests that CTN arises from neurovascular compression, which leads to demyelination and ectopic neuronal firing in the trigeminal nerve (2,(6)(7)(8). However, this explanation fails to capture the complexity and heterogeneity of the disorder, indicating the involvement of additional factors in the development and maintenance of CTN (9)(10)(11)(12)(13)(14).…”
