La neuralgia del trigémino es la enfermedad más común entre los trastornos neuropáticos periféricos en acudir a la consulta neurológica, no tiene preferencia etaria de aparición y su etiología no está clara aún. Se caracteriza por su repentina aparición, un dolor de gran intensidad, que se presenta de manera repetitiva con afectación unilateral y con capacidad de limitar las actividades cotidianas de quien la padece afectando negativamente la calidad de vida.(1,2) El diagnostico oportuno se ha vuelto un reto para el medico clínico y radiólogo, pero gracias a los avances tecnológicos en los últimos años, la prevalencia del mismo ha ido en incremento. Permitiendo al paciente optar por el mejor tratamiento según la etiología de su patología, disminuyendo significativamente los costos por atenciones de emergencia.(3,4) En el presente estudio se mencionan tres casos de pacientes femeninas de diferentes edades que padecen neuralgia del trigémino de etiología primaria, dada por la compresión entre un asa vascular y el nervio, diagnosticados mediante resonancia magnética con la secuencia ponderadas en T2 CISS 3D denominada también como secuencia FIESTA (Fast Imaging Employing Steady-state Acquisition) (5). Se aviva la importancia de la clínica y la sospecha etiológica para una adecuada aplicación de las diferentes secuencias disponibles en resonancia magnética cerebral para un oportuno diagnóstico (6)

Introduction:The study of trigeminal surgery is linked to trigeminal neuralgia and being a serious disorder can impair the patient's quality of life. Objective: To analyze the literature on relevant aspects of trigeminal surgery, such as decompression, sensitivity, anesthesia and schwannomas. Methodology: PubMed, SCOPUS and Google Scholar databases were reviewed to find recent articles published on trigeminal surgery with the following keywords: "trigeminal nerve", "trigeminal surgery", "decompression", "schwannomas", "anesthesia". Results: Microvascular decompression is the most effective procedure for the management of trigeminal neuralgia; highly favourable results have been achieved with this technique. Sensitivity is of great importance in the treatment of this condition, since it is the reason why patients will come for consultation with the aim of eliminating any discomfort related to a nerve condition. Anesthesia requires correct management for a good result in trigeminal surgery and to provide the best care and recovery to patients. Schwannomas are usually benign and rare intracranial tumours. There are several treatments that can help to treat them, the most used and that has achieved excellent results is gamma knife radiosurgery. Conclusions: Trigeminal neuralgia is a condition that usually appears in one or more branches of the trigeminal nerve, trigeminal surgery is usually a method to treat this condition by means of different techniques such as microvascular decompression and thus achieve disappearance of this disorder.

BackgroundClassical trigeminal neuralgia (CTN) is a common and severe chronic neuropathic facial pain disorder. The pathological mechanisms of CTN are not fully understood. Recent studies have shown that resting-state functional magnetic resonance imaging (rs-fMRI) could provide insights into the functional changes of CTN patients and the complexity of neural processes. However, the precise spatial pattern of complexity changes in CTN patients is still unclear. This study is designed to explore the spatial distribution of complexity alterations in CTN patients using brain entropy (BEN).MethodsA total of 85 CTN patients and 79 age- and sex-matched healthy controls (HCs) were enrolled in this study. All participants underwent rs-fMRI and neuropsychological evaluations. BEN changes were analyzed to observe the spatial distribution of CTN patient complexity, as well as the relationship between these changes and clinical variables. Sixteen different machine learning methods were employed to classify the CTN patients from the HCs, and the best-performing method was selected.ResultsCompared with HCs, CTN patients exhibited increased BEN in the thalamus and brainstem, and decreased BEN in the inferior semilunar lobule. Further analyses revealed a low positive correlation between the average BEN values of the thalamus and neuropsychological assessments. Among the 16 machine learning methods, the Conditional Mutual Information Maximization-Random Forest (CMIM-RF) method yielded the highest area under the curve (AUC) of 0.801.ConclusionsOur study demonstrated that BEN changes in the thalamus and pons and inferior semilunar lobule were associated with CTN and machine learning methods could effectively classify CTN patients and HCs based on BEN changes. Our findings may provide new insights into the neuropathological mechanisms of CTN and have implications for the diagnosis and treatment of CTN.
