Microstructural brain abnormalities of children of idiopathic generalized epilepsy with generalized tonic‐clonic seizure: A voxel‐based diffusional kurtosis imaging study

Abstract: The kurtosis parameters (K⊥ , K// , and MK) reflect different microstructural information in the IGE children with GTCS, and this support the value of DKI in studying children GTCS.

“… 43 Therefore, increased Ka and decreased Kr in caudate nucleus result in no significant changes of MK in this region. In gray matter, increased Ka may be due to broken axons, increase of basal dendrites, synaptic refinement, and cell density, which would cause more water diffusion restriction in the axial direction, leading to higher Ka 32 44 45 in the left putamen and caudate nucleus in UD. The water diffusion is more restricted in UD in the left putamen due to the higher Ka in UD than in BD.…”

“…In this type of comparison, with a specific focus on DKI, Hui et al 36 demonstrated that when comparing lesions in subacute patients with stroke injuries with normal-appearing tissue, DKI-derived metrics displayed a higher absolute percent change than DTI-related metrics. In a voxel-based DKI study of children with idiopathic generalized epilepsy and generalized tonic-clonic seizures, Gao et al 39 observed reduced MK in cortical and subcortical regions where MD and FA did not exhibit any alterations. Falangola et al 41 used receiver operating characteristic (ROC) analysis to show that while DTI-related indexes were the best discriminators between Alzheimer's disease and mild cognitive impairment (MCI) patients, MK and RK outperformed DTI-derived indexes in distinguishing between normal controls and MCI.…”

“…Studies have also shown that diffusion tensor imaging (DTI) and diffusion kurtosis imaging (DKI) have important value in the study of pathophysiological mechanisms of epilepsy, lesion focus localization, and so on (Gao et al, 2015). Gaizo et al (2017) used machine learning to classify epilepsy based on diffusion MRI, achieving accuracy of 68% (FA), 51% (MD), and 82% (MK), and he also verified statistically that FA and MK are more significant than MD to diagnose epilepsy.…”
