Microcystic adnexal carcinoma with germinative follicular differentiation

Abstract: Microcystic adnexal carcinoma (MAC) is a low-grade adnexal carcinoma with controversial lines of differentiation. We present here an example of MAC showing histopathologic findings of germinative follicular differentiation in the form of solid aggregates of trichoblastoma intermingled with neoplastic aggregates of MAC. Immunohistochemical findings, showing positivity for PHLDA1 and negativity for BerEp4 in neoplastic aggregates of trichoblastoma, also supported a germinative follicular differentiation. Follicu… Show more

“…MAC is a low-grade malignant neoplasm with a folliculosebaceous-apocrine line of differentiation. 1,2 Clinically, it is a slow-growing neoplasm usually located in the skin of the face, predominantly the nasolabial and periorbital regions, although cases in many other locations have been reported. 1 The indolent clinical behavior of MAC tends to retard the diagnosis, and the gap between the onset of the lesion and the diagnosis is often several years.…”

“…Decapitation secretion is sometimes present in the luminal border of the deeper tubular structures, supporting an apocrine differentiation for MAC. 1 However, cases of MAC with sebaceous 3,4 and follicular differentiation 2,5 have been also described and, therefore, folliculosebaceous–apocrine differentiation may be apparent in examples of this neoplasm. Cytological atypia and mitotic figures are usually absent.…”

“…Microcystic adnexal carcinoma (MAC), a rare and low-grade malignant skin tumor, is characterized by a high rate of misdiagnosis and a preponderance for local recurrence, but seldom seen nodal or distant metastasis. [1] MAC was originally described by Goldstein in 1982, [2] and it is also known as sclerosing sweat duct carcinoma, malignant syringoma, syringoid eccrine carcinoma, eccrine epithelioma, syringomatous carcinoma, sweat gland carcinoma with syringomatous features. [3] Surveillance, Epidemiology and End Results database shows the yearly incidence rate of MAC is 0.52 cases per 1000,000 people, and women are affected more often than men (57.9% vs 42.1%).…”
