Abstract. Gefitinib is a selective epidermal growth factor receptor tyrosine kinase inhibitor utilized for the treatment of advanced non-small cell lung carcinoma. The most commonly reported adverse event during gefitinib therapy is skin rash, particularly a papulopustular acne-like eruption. Cutaneous toxicities can affect treatment compliance and the quality of life of the patient. The present study reports a case of gefitinib-induced atypical skin reaction in a 73-year-old woman with advanced non-small cell lung cancer, who developed a squamous-crusted eruption on her face after 4 weeks of oral treatment with gefitinib at a dose of 250 mg/day. The patient was treated with 100 mg minocyclin (2 tablets/day, orally) and with ryfamicin topically. A complete resolution of the lesions was observed 2 weeks later. The present case report explored the pathogenesis of this skin manifestation, focusing on the underlying immunological mechanisms. A review of the literature concerning skin reactions to gefitinib was also conducted.

Das kolorektale Karzinom ist die zweithäufigste krebsbedingte Todesursache in der westlichen Welt. Das Ziel einer palliativen Therapie ist die Überlebensverlängerung, die Symptomkontrolle und somit die Verbesserung der Lebensqualität. Die Einführung effektiver Chemotherapeutika und monoklonaler Antikörper hat die Prognose von Patienten mit metastasiertem kolorektalem Karzinom in den letzten Jahren entscheidend verbessert, wobei ein mittleres Überleben von nunmehr deutlich über 2 Jahren erreicht werden kann. Die Wahl der Therapie hängt vor allem von der klinischen Situation des Patienten ab: resektable Leber- oder Lungenmetastasen sollen reseziert werden; bei potenziell resektabler Erkrankung soll eine intensive Kombinationstherapie eingesetzt werden, um eine Verkleinerung der Metastasen und so eine mögliche Resektabilität zu erreichen. Ziel ist die Heilung. Bei tumorbedingten Symptomen, Komplikationen oder raschem Progress der Erkrankung ist ebenfalls eine intensive Kombinationschemotherapie indiziert. Neue Therapiestrategien verfolgen den Ansatz, durch gezieltes Absetzen einer, mehrerer oder aller Substanzen Nebenwirkungen einzusparen, therapiefreie Zeit zu ermöglichen und so die Lebensqualität zu verbessern. Dieses Vorgehen wird als «continuum of care»-Ansatz bezeichnet und beschreibt eine auf die jeweilige klinische Situation des Patienten abgestimmte Wahl der Therapie. Sie hat die klassische Therapie in aufeinanderfolgenden Therapielinien abgelöst. Neben der klinischen Situation des Patienten entscheiden mittlerweile molekulare Eigenschaften des Tumors, wie der Mutationsstatus des KRAS-Onkogens, über die Wahl der nunmehr personalisierten Therapie.

Abstract: Empirical oral antibiotic treatment may be performed with oral cephalosporines, ciprofloxacin, levofloxacin or moxifloxacin, as these antimicrobials have high in vitro activity against the majority of the isolated microorganisms and reach high concentrations in the relevant tissue.
