Microbial Keratitis After Descemet Stripping Automated Endothelial Keratoplasty

Abstract: Initial presentation of microbial keratitis after DSAEK may involve either the host or the posterior lamellar disk alone. A microvitrectomy scissors through the side port may be used for biopsy of posterior lamellar disk in recalcitrant infection.

“…Although significant controversy exists regarding the value and form of prophylactic antifungal therapy, 5 almost all established interface infections have failed systemic and/or topical antifungal therapy most commonly requiring penetrating keratoplasty or even leading to the loss of the eye. 3,4,[6][7][8] We present an alternative medical therapeutic technique that can avoid the longand short-term risks of urgent keratoplasty and preserve a potentially viable endothelial graft.…”

“…There have been clinical case reports of fungal infection deep in the interface after DSAEK [1], and bacterial contamination has recently been described, as well [2,3]. Although the two types of infection may appear similar, the timing of the infection may be the best indication as to the specific cause.…”

“…They state that microvitrectomy scissors through the side port may be used for biopsy of posterior lamellar disc in cases of recalcitrant infection. 8 Ortiz-Gomariz et al 9 report on clinical and confocal microscopy features of late-onset C. albicans keratitis after DSAEK. In their report, a 76-year-old woman underwent a routine DSAEK procedure for decompensating cornea due to Fuchs cornea dystrophy.…”
