Цель исследования-оценка влияния метилпреднизолона на синдром капиллярной утечки при операциях в условиях нормотермического искусственного кровообращения. Материал и методы. Обследован 21 пациент с внутривенным введением после индукции анестезии метилпреднизолона в дозе 20 мг/кг массы тела (основная группа) и 21 пациент без применения метилпреднизолона (контрольная группа). Оценивались уровни креатинина, альбумина, цистатина С, sЕ селектина, эндотелина 1, глюкозы в крови, креатинина в моче, микроальбуминурии, газовый состав крови и пери операционные результаты. Результаты. Уровни микроальбуминурии и отношения альбумина к креатинину в моче в группах статистически значимо не различались. В группе с применением метилпреднизолона при сравнении с кон трольной группой на этапах исследования выявлено увеличение потребности в инотропной поддержке, коэффициен та венозного шунтирования крови, уровней гликемии, эндотелина 1, sЕ селектина, интерлейкина 10, цистатина С, а также уменьшение скорости клубочковой фильтрации, РаО 2 /РАО 2 и PaO 2 /FiO 2 , уровня ИЛ 6. Заключение. Опреде лено, что применение метилпреднизолона не уменьшает проявлений синдрома капиллярной утечки. Введение пульс дозы глюкокортикостероидов способствовало усугублению явлений эндотелиальной дисфункции (таких, как увели чение уровней эндотелина 1 и sЕ селектина на этапах исследования) и отрицательно влияло на восстановление легочной и почечной функций после искусственного кровообращения. Objective: to evaluate the effect of methylprednisolone on capillary leak syndrome at surgery under normothermic car diopulmonary bypass. Subjects and methods. Twenty one patients who received intravenous methylprednisolone in a dose of 20 mg/kg body weight after anesthesia induction (a study group) and 21 patients who were not given this agent (a con trol group) were examined. The investigators estimated the levels of creatinine, albumin, cystatin C, sE selectin, endothe lin 1, and glucose in blood, creatinine in urine, microalbuminuria, and blood gas composition and assessed perioperative outcomes. Results. There were statistically significant differences between both groups in microalbuminuria level and albumin/creatinine ratio. At the study stages, there were increases in inotropic support needs, blood venous shunt ratio, and the levels of glucose, endothelin 1, sE selectin, interleukin 10, and cystatin C and decreases in glomerular filtration rate, PaO 2 /PAO 2 and PaO 2 /FiO 2 , and IL 6 level in the methylprednisolone group as compared with the controls. Conclusion. The use of methylprednisolone was not ascertained to reduce the manifestations of capillary leak syndrome.
