Methotrexate-induced panniculitis in a patient with rheumatoid arthritis

Abstract: Methotrexate-induced accelerated nodulosis (MIAN) is not an uncommon adverse effect associated with the use of the methotrexate in rheumatoid arthritis. Limited case reports describe panniculitis as a pathological finding in this setting. A 31-year-old female with seropositive rheumatoid arthritis on methotrexate therapy presented with a 2-week history of sudden onset of painful infiltrated subcutaneous nodules on both forearms. Based on clinical and histological findings, a diagnosis of methotrexate-induced p… Show more

“…Adverse effects associated with the use of MTX additionally include the development of accelerated nodulosis, also known as MTX-induced accelerated nodulosis (MIAN), which occurs in (1–10)% of patients on MTX. 86 However, most adverse effects can be reversed by supplementation with calcium or sodium folinate. 83 …”

“…They can appear on the forearms and elbows and even in internal organs. 6 , 7 In nodosity accelerated by methotrexate, the histological alterations resemble those found in the rheumatoid nodule. In the patient in question, this hypothesis was rejected based on the histopathology.…”

“…13 Since MTXinduced accelerated nodulosis and panniculitis are linked to patients expressing HLA-DRB1, it is important to note there is a certain genetic susceptibility present in these patients. 6,7 Therefore, the hypothesised mechanism of MTX's role in the pathogenesis of SURN could be similar.…”

“…6 Furthermore, additional evidence suggests a correlation between MTX-induced accelerated nodulosis and subsequent panniculitis in these susceptible individuals. 7 Superficial ulcerating necrobiosis, a rare entity that may accompany RA, can develop to become superficial ulcerating rheumatoid necrobiosis (SURN). 8 Necrobiosis is a generalised descriptor that refers to skin without connective tissue fibres upon histological examination Histologically, these lesions are characterised by bordering palisading histiocytes, multinucleated cells, and epithelioid cells.…”
