Methotrexate and Rheumatoid Arthritis: Current Evidence Regarding Subcutaneous Versus Oral Routes of Administration

Abstract: Methotrexate (MTX) is still considered the drug of choice in rheumatoid arthritis (RA) management. Comparing subcutaneous (MTX SC) and oral (MTX OR) routes of administration is important to optimize the everyday therapeutic strategy in the real-life setting. This review summarizes scientific evidence currently available on this topic. As shown by pharmacokinetic studies, at the same dose level, bioavailability of MTX SC is significantly higher and less variable than that of MTX OR. This difference is even more… Show more

“…When discussing the route of MTX administration, several studies recommend the subcutaneous use of MTX over oral use in order to improve its efficacy and/or to reduce its side‐effects . In our study, MTX was administered orally for most patients ( n = 74, 84%).…”

“…Longer monotherapy duration in older patients may be due to less aggressive treatment of RA in elderly patients (18)(19)(20)(21). We also found that the use of SC MTX was associated with increased frequency of GI symptoms, contrary to the findings of other analyses (22). This may be related to the higher bioavailability of SC MTX when compared to oral MTX or other confounders such as additional medications or illnesses that we were unable to account for in our model (15,23).…”

“…RA can be effectively managed by anti-rheumatic drugs (such as, Methotrexate (MTX), leflunomide, sulfasalazine, and hydroxy-chloroquine [14] combined with/without different non-steroidal anti-inflammatory drug (aspirin, celecoxib, diclofenac, ibuprofen, indomethacin, ketoprofen etc.,) among these drugs, MTX was considered as an effective drug since 1980 [15]. Inspite, we are also developing a formulation using MTX with another drug combination for the effective treatment of RA.…”
