Methods of determining the relationship of the mandibular canal and third molars: a survey of Australian oral and maxillofacial surgeons

Abstract: whether the presurgical determination of proximity and position (buccal or lingual) of the canal utilizing CT has any usefulness in determining the surgical protocol or affect on postoperative morbidity.Key words: OMF, third molars, mandibular canal, inferior alveolar nerve. Abbreviations and acronyms: ANZOMS = Australian and New Zealand Association of Oral and MaxillofacialSurgeons; CT = computed tomography; IAN = inferior alveolar nerve; MC = mandibular canal.

“…[10] Radiographic signs, detectable on the panoramic radiography that indicate the presence of a close relationship between the inferior alveolar nerve and the lower third molar are [11,12]: -Radiotrasparent band darkening the root of the third molar because of the decreasing of bone density produced by the mandibular canal crossing the area. -Interruption of the line marking the roof of the canal due to the root of the third molar crossing it.…”

“…Koong and coworkers prove in their study that in Australia the majority of Oral and maxillofacial surgeons rely on panoramic radiographs for determining the relation of mandibular third molars with the mandibular canal. However, a relatively small percentage of surgeons considered that data from panoramic radiographs has insignificant accuracy for this purpose (12).…”

“…The literature states as a method of radiographic reference -the orthopantomography (panoramic radiography, OPG), for which clinicians use a series of radiographic markers for intimate relation of the molars to the MC (11,12).…”

“…With this simple technique, the operator can attempt to remove many third molars which on presurgical evaluation satisfy criteria given by Rood and Shehab [5] or pose to be a risk factor for IAN injury [6][7][8].…”
