Metatarsophalangeal joint pain in psoriatic arthritis: a cross-sectional study

Abstract: Objective. The aim of this study was to identify independent predictors of pain at the MTP joints in patients with PsA.Methods. Thirty-four consecutive patients with PsA (mean age 45.3 years, 65% female, mean disease duration 9.9 years) and 22 control participants (mean age 37.9 years, 64% female) underwent clinical and US examination to determine the presence of pain, swelling, synovitis, erosions, effusions and submetatarsal bursae at the MTP joints. Mean barefoot peak plantar pressures were determined at ea… Show more

“…Apart from chondrocytes, other pathogenic factors, such as immune cells, cytokines, and hormones, lead to the pathogenic changes in the subchondral bone. Increasing evidence has demonstrated that synovium inflammation is involved in the progression of OA and associated with joint pain and dysfunction [ 41 , 42 ]. Various types of immune cells are infiltrated in the inflamed synovium of OA patients.…”

“…Synovitis has been related not only to knee pain but also to knee joint function, using objective outcome measures of walking and stair-climbing times (72,73). These efforts have provided substantial evidence that synovitis is associated with major symptoms such as pain and degree of joint dysfunction, and may promote more rapid cartilage degeneration (69,74).…”

“…There is evidence on the link between PE and US to assess synovitis in small joints [ 14 ]. Turner et al investigated the US findings on the foot in PsA and found that higher body mass index (BMI), female sex, subluxation, and erosions were independent predictors of metatarsophalangeal (MTP) joint pain in PsA patients in addition to US-detected synovitis [ 15 ]. They clinically found 129 (32%) painful and 10 (3%) swollen MTP joints, while US-synovitis and PD positivity was less often, detected in 47 (14%), and 6 (2%) in MTP joints.…”
