460W.D~ex: 6. Die Tetanusprophylaxe mit aktivem Impfstoff kann nur zum Zeitpunkt der Wahl durchgefiihrt werden. Eine solche Aufgabe ist allein durch die Organe der Gesetzgebung zu 15sen.7. Nur die Immunisierung der Gesamtbevflkerung beseitigt alas Tetanusdilemma. Dennoeh ist eine schrittweise Einffihrung der aktiven Immunisierung vertretbar. Es wird empfohlen, mit der Immunisierung der Schwangeren, der Kleinst-und Schulkinder zu beginnen.8. Der Kostenaufwand ist offenbar das Hauptargument gegen die gesetzliche Einffihrung der Immunisierung. Dem wird entgegengehalten, da$ die Serumprophylaxe allein j~hrlich eine Geldaufwendung beansprucht, mit der 2 Geburtsjahrgange aktiv immunisiert werden kfnnen. Da naeh den neuesten Ergebnissen der Wissenschaft die Dosis bei der Serumprophylaxe erhfht werden mug, da ferner die Anwendung h6her konzentrierter und gereinigter Sara angestrebt werden mug, wird sich in Zukunft der Kostenaufwand ffir die Serumprophylaxe noch vergrfgerm 9. Die aktive Immunisierung ist abet nieht nut bflliger als die Gesamtheit der bisher gegen Tetanus gerichteten MaBnahmen. Sie ist aueh das einzige Verfahren, das eine Ausrottung dieser furchtbaren Krankheit ermfglieht.10. Die Tatsaehe einer immer wieder erfolglosen Mabnung gegenfiber dem Gesetzgeber zwingt uns zu folgender Feststellung : ,,Wir ~rzte lehnen die Verantwortung an den Toten, die eine der /urchtbarsten Krankheiten in gleichbleibender Anzahl jiihrlich /ordert, ab und sehen uns gen6tigt, die Bev6lkerung au/zukldiren. Es ist #rner keinem Arzte zuzumuten, daft er die Sorge um die Tetanusverhi~tung bei #dem Frisehverletzten allein tri~gt. Die U~terlassung einer der iibliehen, im Einzel/alle aber doch nicht zuverliissigen Ma/3nahme als Versto[3 gegen die dirztliche Sorg/altspflicht anzusehen oder gar gerichtlich zu ahnden, lehnen wit 9rundsiitzlich ab." Obwohl die Behandlung soleher l~iickfglle wenig dankbar ist, sollte sich der gewissenhafte Chirurg doeh die Frage vorlegen, ob er nieht in Einzelf~llen verpfliehtet sei, die l~ettung eines Riiekfallkrebskranken dureh einen Zweiteingriff zu versuehen.Den Entsehlug zur Zweitoperation wird er aber nur fassen k6nnen, wenn er zur Uberzeugung gekommen ist, dal~ ein Krebsriiekfall noch nieht endgiiltig das Sehieksal besiegelt. Nach der pessimistisehen Einstellung vieler Chirurgen miiBte man dies allerdings vermuten, und man
