Metaplastic synovial cyst of the skin is a recently recognized entity characterized by an intradermal nodule that usually occurs at the site of previous surgical trauma. Histologically, the lesion demonstrates a cystic structure with villous-like projections and a lining resembling hyperplastic synovium. We have studied two patients with rheumatoid arthritis, aged 46 and 55 years, who presented with cystic nodules localized on the thumb and great toes, respectively, without any history of previous trauma or surgical procedures performed in the areas. The presence of vimentin and CD 68 positivity of the cells lining the cyst walls supports the similarities between normal and metaplastic synovium. We hypothesize that constant pressure on the great toe, repeated manipulation of the finger, and chronic inflammation around the affected joints may have played roles in the pathogenesis of the lesions in our patients.

A cutaneous metaplastic synovial cyst (CMSC) is a cyst lined with metaplastic synovial tissue, which includes the formation of an intracystic villous structure resembling hyperplastic synovial villi. Clinically, the lesion is a tender, subcutaneous nodule that usually occurs at the site of previous surgical trauma and is frequently misdiagnosed as a suture granuloma. The actual cause remains unclear; however, trauma is presumed to be a precipitating factor, as most reported cases have demonstrated a history of antecedent cutaneous injury. Here, we present a case of CMSC in a 51-year-old woman who presented with a cystic mass localized in the left sole. She had no history of previous trauma or surgical procedures performed in the area. Although the case explained in this report is a spontaneous case of CMSC that occurred without a history of trauma, it is believed to have been caused by constant and chronic pressure since CMSC occurred in the first metatarsal head area, a part of the sole where heavy pressure is consistently applied.

Exogenous and endogenous substances can produce some interesting cutaneous abnormalities. Exogenous lesions are externally introduced materials or foreign bodies. Endogenous lesions are substances that result from tissue host response to an injury or physical insult. There may be an overlap of these endogenous and exogenous lesions such that externally introduced materials can lead to peculiar tissue reactions. Pathologists should be aware of these anomalies as many of them can simulate neoplasms (ie, Monsel's reaction, polyvinylpyrrolidone disease, silica reaction) and inflammatory (ie, paraffinoma) and infectious processes (ie, Drysol(R) reaction and Lycopodium granuloma). The most interesting and important entities within each of these categories are presented with essential clinical and pathologic descriptions.
