Metaplastic Matrix-Producing Carcinoma and Apocrine Lobular Carcinoma In Situ Associated with Microglandular Adenosis: A Unique Case Report

Abstract: Microglandular adenosis is a non-lobulocentric haphazard proliferation of small round glands composed of a single layer of flat to cuboidal epithelial cells. The glandular structures lack a myoepithelial layer; however, they are surrounded by a basement membrane. Its clinical course is benign, when it is not associated with invasive carcinoma. In around 30% of cases, there is a gradual transition to atypical microglandular adenosis, carcinoma in situ, and invasive breast carcinoma of several different histolog… Show more

“…Besides a malignant phyllodes tumor, some breast tumors with components of spindle cells with myxoid matrix or spindle cells with giant cells have to be listed for the differential diagnosis. These include myxofibroblastoma, and metaplastic carcinoma [22][23][24]. Especially, the exclusion of metaplastic carcinoma is very important because of the difference in the treatment strategy in advanced cases [25].…”

“…In mixed ILC and invasive mucinous carcinoma, immunostaining for E-cadherin will delineate the two different immunophenotypes. MMPC [ 41 ] and polymorphous mammary adenocarcinoma [ 42 , 43 ] also resemble ILC with extracellular mucin production since the former has a mucoid/matrix stroma, and the latter may simulate the single-file pattern of ILC [ 42 , 44 ]. Immunohistochemically, they may lack E-cadherin expression [ 42 , 45 ].…”

“…Immunohistochemically, they may lack E-cadherin expression [ 42 , 45 ]. Moreover, both tumors lack ER expression, while MMPC also shows positive staining for S-100 with rare exceptions [ 41 , 46 ]. Finally, one should also not forget the possibility (albeit rare) of metastatic signet-ring cell carcinomas to the breast [ 25 ].…”

“…MSCC and fibromatosis-like metaplastic carcinoma may simulate the histological features of MFB. Also, in rare cases with cartilaginous or osseous components, MMPC may enter the differential diagnosis [ 30 , 31 ].…”
