Metachronous isolated locally advanced pancreatic metastasis from chromophobe renal cell carcinoma

Abstract: Highlights Isolated pancreatic metastasis from renal cell carcinoma is relatively rare and it's usually seen in clear cell renal cell carcinoma (CCRCC), but its occurrence from chromophobe renal cell carcinoma is extremely rare. Pancreatic location is often diagnosed during routine surveillance imaging for the primary lesion. Patients in whom the pancreas is the only metastatic site and who are fit enough to undergo pancreatic surgery appear to be good can… Show more

“…From 421 cases reported casuistically, a cumulative 5-and 10-year survival rate (SR) of 75.7% and 48.4%, respectively, can be calculated (Figure 2). In single and multicentre institutional reports of isolated PM [99,143,183,185,191,201,209,216,234], comparable results were reported with five-year SR of 52%-84%, 53% [99], 61% [143], 52% [183], 72% [184], 56% [190], 71% [200], 69% [208], 80% [215], 84% (Median 72%) [233]. 10a: 63% (median 69%) [254].…”

“…In Figure 2, we also present the results of the few observations that were not administered surgically or targeted therapy (TKI, m-tor inhibitors or immunotherapy). The three-year SR of 56% calculated from 19 casuistically reported observations [10,22,63,64,78,90,107,166,175,185,188,194,216,250] is a favourable result for the spontaneous course of a metastatic RCC. However, the result is significantly worse than after interventions with curative intent (p = 0.013).…”

“…The site and frequency of metastatic involvement depends on the histologic subtype of RCC—clear cell (ccRCC), papillary (pRCC), spindle cell type (sRCC) and chromophobe (chrRCC). PM are most frequently seen in ccRCC [ 31 , 32 , 33 ]. PM from RCC have also been reported in ectopic pancreatic tissue, such as in the case reported by Yano et al [ 34 ].…”

“…ChRCC develops from dark cells in the collecting duct epithelium of the kidney [ 15 ]. There was 89.3% recurrence-free survival (RFS) and 93% cancer-specific survival (CSS) rates for chRCC after 5 years [ 16 ]. Metastatic disease accounts for only 6% of chRCC patients [ 17 ].…”
