Obesity is a condition in which excess body fat accumulates due to lipids producing adipocytes and an increased number of differentiated mature cells. Recently, new findings have shown that macrophages infiltrate into adipose tissues and produce various pro-inflammatory cytokines in obese subjects. The inflammatory changes induced by the cross-talk between adipocytes and macrophages are critical for the pathophysiology of obesity and thus of metabolic syndrome. Caffeic acid phenethyl ester (CAPE) is known to have many functions, including antibacterial, anticancer and anti-inflammatory properties, but there is no evidence of its effect on the inflammatory responses in hypertrophic adipocytes through stimulation by macrophages. We investigated the effect of CAPE on macrophages and hypertrophic adipocytes in this study. CAPE significantly suppressed the levels of lipopolysaccharide (LPS)-induced interleukin (IL)-1-beta, tumor necrosis factor (TNF)-alpha and monocyte chemoattractant protein (MCP)-1 from a macrophage cell line, RAW264.7. Supernatants of stimulated RAW264.7 macrophages drastically increased mRNA levels of pro-inflammatory cytokines such as IL-6, MCP-1 and TNF-alpha in 3T3-L1 hypertrophic adipocytes. CAPE also significantly and dose-dependently reduced the gene expression of these cytokines. Our findings indicate that CAPE has inhibitory effects on the production of pro-inflammatory cytokines from LPS-stimulated RAW264.7 macrophages. In addition, CAPE suppressed gene expressions of cytokines under inflammatory conditions of hypertrophic adipocytes, suggesting that it may have the potential to suppress inflammation by macrophage infiltration into adipose tissue in obese patients.Key words caffeic acid phenethyl ester; hypertrophic adipocyte; macrophage; pro-inflammatory cytokine Obesity is a major risk factor for metabolic syndrome, which includes a number of disorders, such as hypertension, heart diseases and diabetes.1,2) Obesity is a condition in which excess body fat accumulates due to lipids producing hypertrophic adipocytes and an increased number of differentiated mature cells. Findings of the preliminary survey showed a high proportion of geriatric people in Dusun Mergan, Sendangmulyo, Sleman, which leads to high risk of cardiovascular and other degenerative diseases. This community service program aimed to identify the health status of Mergan's people through health screening and to help them to maintain their health. This program was conducted in collaboration between the Faculty members and students of Pharmacy Faculty of Universitas Sanata Dharma Yogyakarta (USD). There were two consecutive periods; the first was in 7th of May and the second was in 22nd of October 2018. Health education, health screening, and health examination were conducted in each period. There were 93 people involved in the first period and 64 people in the second; however, only 32 persons who completely attended the two meeting periods. The program resulted data of 60.7% Mergan's people with a risk of dyslipidemia, and 59.0% with hyperglycemic. There was no significant difference between the first and the second periods of examination of glucose level, cholesterol, and uric acid. Therefore, it is urgent to educate people in Mergan Village to promote and maintain a healthy lifestyle in order to decrease morbidity of degenerative disease in the community.

BackgroundDisturbances of the fatty acids composition in plasma and red blood cells and eicosanoid synthesis play an important role in the metabolic syndrome (MS) formation.MethodsThe observation group included 61 people with metabolic syndrome (30 patients with MS and normal levels of insulin, 31 people with MS and insulin resistance - IR). The parameters of carbohydrate and lipid metabolism in blood serum were examined. The composition of nonesterified fatty acids (NEFA), fatty acid (FA) of red blood cells lipids was analyzed by gas-liquid chromatography. Eicosanoids level in MS patients blood serum was studied by enzyme immunoassay.ResultsIn MS patients in the absence of glucose-insulin homeostasis disturbances and in patients with IR the accumulation of polyunsaturated fatty acids (18:2 n6, 18:3 n3, 22:4 n6) and lower pool of saturated FA (12:0, 14:0, 16: 0, 17:0) in plasma were discovered. A deficit of polyunsaturated FA (18:3 n3, 20:4 n6) with a predominance of on-saturated FA (14:0, 18:0) in erythrocyte membranes was revealed. In MS patients regardless of the carbohydrate metabolism status high levels of leukotriene B4 and 6-keto-prostaglandin-F1α in serum were found. The development of IR in MS patients leads to increased synthesis of thromboxane A2.ConclusionThe results revealed a disturbance in nonesterified fatty acids of plasma lipids and red blood cells, eicosanoid synthesis in MS patients. The breach of the plasma and cell membranes fatty acids compositions, synthesis of vasoactive and proinflammatory eicosanoids is an important pathogenetic part of the MS development.
