Background: The aims of this study are to evaluate the efficacy and safety of intragastric balloon (IB) to reduce the weight and body mass index (BMI) in severely obese adolescent females and to describe the changes in the liver enzymes and lipid and glucose metabolism biomarkers. Methods: This study included 10 severely obese post-menarche adolescent females. We evaluated anthropometric data, lipid profile, glucose metabolism biomarkers, and liver enzymes before insertion and after removal of the IB. Results: BMI and weight reduction were larger during the first month of intervention. Especially in the first week, there was a reduction of 1.74 ± 0.46 kg/m 2 (p = 0.004) and 6.46 ± 1.52 kg (p = 0,002), respectively. After 3 months, there was an average BMI reduction of 4.29 ± 1.04 kg/m (p = 0.005) and weight reduction of 12.9 ± 3.08 kg (p = 0.004). From the initial moment to study conclusion, there was a statistically significant reduction in insulin levels (9.0 ± 2.8 U/mL; p = 0. 012) and in homeostatic model assessment-insulin resistance (2.0 ± 0.6; p = 0.009). Five patients reported, during the first week, epigastric pain; nausea and vomiting were observed in two patients. No adolescents presented dysphagia during IB use. Conclusion: IB use in adolescent females with severe obesity in association with a conservative multidisciplinary treatment had a positive impact in BMI reduction, with few adverse effects. There was also an improvement in insulin resistance.

Objective: There are limited controlled data for intragastric balloons (IGB) in obesity treatment. This randomized, controlled study evaluated the efficacy and safety of an IGB in obese individuals with metabolic syndrome (MS). Design and Methods: Sixty-six adults (BMI: 30-40 kg/m 2 ) were randomized to IGB for 6 months, with a 12 month behavioral modification (IGB Group; ''IGBG''), or 12 month behavioral modification alone (Control Group; ''CG''). The primary outcome was percentage change in body weight. Results: Thirty-one subjects (female: 68%; mean age: 43; mean BMI: 36.0) were randomized to IGBG and 35 (66%; 48; 36.7) to CG. At 6 months, there was a significantly greater weight loss in the IGBG: À14.2 vs. À4.8; P < 0.0001. This was associated with a significantly greater reduction in waist circumference, and an improvement in quality of life, with a trend for a larger %MS remission (50% vs. 30%; n.s.). At month 12, the differences in weight loss were enduring: À9.2 vs. À5.2; P ¼ 0.007. Gastrointestinal-related adverse events were common in the IGBG, resolving predominantly within two weeks. The IGB was removed prematurely in three subjects (one for refractory gastrointestinal symptoms). Conclusions: Statistically significant and clinically relevant improvements in weight loss and health outcomes were observed with the IGBG at 6 months versus behavioral modification alone. The differential weight loss was still evident 6 months after IGB removal.

Obesity is the most common disease of affluence of the XXI century. According to WHO (World Health Organization), it is defined as a chronic metabolic disorder manifested by excessive accumulation of adipose tissue with high tendency for familial occurrence. According to WHO, obesity reaches epidemic proportions in many countries. High BMI (Body Mass Index) correlates with coexisting diseases. Traditional dietetic treatment often does not bring any results. A form of conservative (non-surgical) support for patients in fighting with obesity is the reduction of stomach volume by bioenteric intragastric balloon (BIB) treatment. The aim of the work was to develop a diet with anti-inflammatory properties, well-tolerated by the patients after BIB treatment. An American diet was modified by changing the composition of fatty acids, increasing anti-oxidative potential and adding synbiotics for patients treated with BIB. Chemical analysis of reconstructed food ratios of recommended diet was performed, analysing the content of micronutrients, composition of fatty acids, antioxidative capacity, reducing power and the content of polyphenols. Improvement in anthropometric measurement results and satisfying body weight loss were obtained, while preserving fat-free body mass. Improvement in the parameters of lipid metabolism was also observed, that is, decrease in total CH (cholesterol) and TG (triglycerides), and normalized concentration of HDL (high density lipoproteins) and LDL (low density lipoproteins) fractions. Reduced concentration of glucose in blood and lower blood pressure was also noted. Performed study confirms the effectiveness of complex treatment with BIB and properly adjusted individualized diet. Observations and own experience allow to deduce that patients who resign from systematic contact with a dietician cannot maintain reduced body weight. Abandoning previous habits is the only way to maintain the effect of weight loss. Most importantly, the change in patients’ awareness and consequent behaviour in the future are crucial. Even though genes may contribute to obesity, environmental factors mainly determine the possibility of the disease to occur. Therefore, the change of patients’ lifestyle after body weight reduction will decide on their fate.
