Metabolic Acidosis with Ophthalmic Dorzolamide in a Neonate

Abstract: Carbonic anhydrase inhibitors are a common cause of normal anion gap metabolic acidosis; however, development is less commonly associated with ophthalmic administration of these agents. We report a case of a premature neonate who was being treated at our institution with betaxolol, dorzolamide, and latanoprost ophthalmic products for suspected bilateral congenital glaucoma. In addition, the patient was also receiving caffeine, ursodiol, and acidified liquid human milk fortifier. The patient developed a normal … Show more

“…Sight. CA inhibitors applied as eye drops have long been used to treat glaucoma by virtue of their ability to reduce the production of aqueous humor, although even their localized ophthalmic use has been documented to lead to the side-effect of systemic MAc in some prone individuals [144] , [145] . Corneal edema that results from the expression of mutant misfolded SLC4A11 may be amenable to correction by small molecule folding chaperones [146] .…”

“… 51 , 52 A case of normal anion gap metabolic acidosis in a premature neonate who was on topical dorzolamide, betaxolol, and latanoprost for bilateral congenital glaucoma has also been reported. 53 Although upon discontinuation of the dorzolamide ophthalmic solution, the metabolic acidosis gradually resolved. Thus dorzolamide was suspected to be the primary cause of a normal gap acidosis.…”

“…Warsham et al [7] published the case report of a newborn with sacrococcygeal teratoma whose mother used acetazolamide throughout her pregnancy. There is also a risk of metabolic disorders in newborns if the mother used acetazolamide whilst pregnant [8].…”
