Metabolic acid-base adaptation triggered by acute persistent hypercapnia in mechanically ventilated patients with acute respiratory distress syndrome

Abstract: ObjectiveHypercapnia resulting from protective ventilation in acute respiratory distress syndrome triggers metabolic pH compensation, which is not entirely characterized. We aimed to describe this metabolic compensation.MethodsThe data were retrieved from a prospective collected database. Variables from patients' admission and from hypercapnia installation until the third day after installation were gathered. Forty-one patients with acute respiratory distress syndrome were analyzed, including twenty-six with p… Show more

“…Second, severely and critically ill patients, who are frequently affected by organ dysfunction and often receive multiple medications that affect K + regulation, are at high risk of developing abnormal K + levels 20 . Third, abnormal plasma potassium may be one of the manifestations of acid-base balance disorder, which suggests severe acute respiratory distress syndrome 21 .…”

“…Abnormal plasma potassium levels may be one of the symptoms of acid-base imbalance that occurs in patients with acute respiratory failure. It may also cause cardiac arrhythmia, bradyarrhythmia, complete heart block and circulatory arrest [43,44]. In a study concerning critically ill COVID-19 patients, Shengcong et al found a significant increase in mortality rate in patients with potassium levels ≥5.0 mmol/L [45].…”

“…Abnormal plasma Potassium levels may be a symptom of an acid–base imbalance in patients suffering from acute respiratory failure. Markedly, it can also result in cardiac arrhythmia, Brady arrhythmia, complete heart block, and circulatory arrest 56 , 58 . Previous research has examined the link between Potassium and mortality in the intensive care unit.…”
