Meta-analysis of the results of intravenous gamma globulin treatment of coronary artery lesions in Kawasaki disease

Abstract: The objective of this study was to evaluate the efficacy of intravenous gamma globulin (IVGG) therapy on the prevention of coronary artery lesions (CALs) in patients with Kawasaki disease (KD), with reference to the literature on meta-analyses in randomized controlled studies. Studies from 1984 to the end of 2000 obtained from the National Library of Medicine or from the bibliographies of these articles were used in the analysis. The total number of patients with KD covered in 17 articles was 4020. All the art… Show more

“…The mainstay of initial treatment for both complete and incomplete KD is a single high dose of IVIG together with acetylsalicylic acid (ASA), which is supported by clinical trial evidence. 127,157 This section covers treatment from onset of the acute illness until resolution of acute systemic inflammation and when coronary artery luminal dimensions have stabilized and are no longer expanding.…”

“…126,158,159 Meta-analyses of IVIG compared with placebo have shown a conclusive decrease in new coronary artery abnormalities among IVIG-treated patients. 157,160 The mechanism of action of IVIG in treatment of KD is unknown. IVIG appears to have a generalized anti-inflammatory effect.…”

“…Even with prompt IVIG therapy, up to 20% of children will develop recurrent or persistent fevers. These children are termed IVIG-resistant [61][62][63]. There are several risk factors for IVIG-resistant KD including delayed initial IVIG administration, increased ESR, decreased hemoglobin and platelet levels, oral mucosal alterations, cervical lymphadenopathy, extremity swelling, and polymorphous rash [64•].…”

“…When compared with the CAL incidence of 5% in treated patients, our result had a lower incidence, though the difference was not statistically significant. In a meta-analysis of IVIG results, the incidence of CALs was 10.8% at 30 days and 2.8% at 60 days after the onset of treatment 17). Compared with these previous studies, IVIG-SN was not inferior.…”
