Meta‐analysis: combination therapy with interferon‐α 2a/2b and ribavirin for patients with chronic hepatitis C previously non‐responsive to interferon

Abstract: SUMMARYBackground: The efficacy of interferon-a plus ribavirin treatment for patients not responding to interferon monotherapy is not well established. Aim: To assess the efficacy and safety of combination therapy with interferon-a 2a ⁄2b plus ribavirin by performing a meta-analysis of randomized clinical trials. Methods: A systematic search of electronic databases for randomized clinical trials of interferon-a 2a ⁄2b plus ribavirin was conducted independently by two investigators. Data abstraction was perform… Show more

“…Each of these reviews included only RCTs in their primary analyses. Two reviews (Cheng 77 and San Miguel 80 ) used more stringent inclusion criteria with regard to doses of IFN and RBV, dose frequency and treatment duration. The other three reviews did not restrict inclusion based on these criteria.…”

“…77,80 In the Cheng review 77 SVRs were determined after excluding studies containing the highest or lowest proportion of patients with the covariate. The overall SVR for trials that included more than 50% of patients with genotype 1 was decreased compared with the primary analysis.…”

“…However, these were tolerated. About 14% of patients withdrew from this study because of adverse events, which is a higher rate than that in treatment with interferon‐ α [8]. It was possibly related to the higher dose of interferon‐ β used in this study.…”

“…8 Re-treatment with similar regimen after the initial failure is not an optimal choice as the outcome is relatively poor. 9,10 Furthermore, the IFN-α based treatment is not without side effects. Patients often have to discontinue treatment due to intolerable side effects or reduce the treatment dosages to minimize such effects.…”
