Mesenchymal Stem Cell-Derived Exosomes: Toward Cell-Free Therapeutic Strategies in Chronic Kidney Disease

Abstract: Chronic kidney disease (CKD) is rising in global prevalence and has become a worldwide public health problem, with adverse outcomes of kidney failure, cardiovascular disease, and premature death. However, current treatments are limited to slowing rather than reversing disease progression or restoring functional nephrons. Hence, innovative strategies aimed at kidney tissue recovery hold promise for CKD therapy. Mesenchymal stem cells (MSCs) are commonly used for regenerative therapy due to their potential for p… Show more

“…Subsequent studies have also confirmed that MSCs alleviate other animal models of AKI induced by ischemia–reperfusion, glycerol, sepsis, cecal ligation and puncture 162 , 163 , and so on. In addition, MSCs transplantation significantly alleviated CKD progression by inhibiting renal tubular epithelial cell apoptosis, EMT process, and inducing cell autophagy 15 , 164 . For example, Liu et al 165 showed that treatment with BMMSCs restricted inflammation and renal damage in the IRI model.…”

“…Recent studies have shown that EVs secreted by MSCs have similar therapeutic effects on MSCs because some of the paracrine effects of MSCs are derived from exosomes, and most of the therapeutic effects of MSCs are responsible for the paracrine effects of EVs in certain diseases via miRNAs, mRNAs, and proteins as functional molecules [ 212 , 213 , 214 , 215 ].…”

“…Moreover, stem cells may have potential carcinogenicity, which greatly limits their clinical applications (Hickson et al, 2016). Emerging evidence confirms that MSC-exosomes can be used as natural carriers for targeted drug delivery (Cao et al, 2022). Thus, therapeutic agents can be efficiently incorporated into exosomes and then delivered to diseased tissues.…”
