Background: Methylmercury (MeHg) is an environmental pollutant of global public health concern. MeHg is associated with immune dysfunction but the underlying mechanisms are unclear. The most common route of MeHg exposure is through consumption of fatty fish that contain beneficial n-3 polyunsaturated fatty acids (PUFA) that may protect against MeHg toxicity. Objectives: To better inform individual costs and benefits of fish consumption, we aimed to identify candidate epigenetic biomarkers of biological responses that reflect MeHg toxicity and PUFA protection. Methods: We profiled genome-wide DNA methylation using Illumina Infinium MethylationEPIC BeadChip in whole blood from N=32 individuals from Madre de Dios, Peru. Madre de Dios has high artisanal and small-scale gold mining activity, which results in high MeHg exposure to nearby residents. We compared DNA methylation in N=16 individuals with high (>10 μg/g) vs. N=16 individuals with low (<1 μg/g) total hair mercury (a proxy for methylmercury exposure), matched on age and sex. Results: We identified hypomethylated (i.e., likely activated) genes and promoters in high vs. low MeHg-exposed participants linked to Th1/Th2 immune imbalance, decreased IL-7 signaling, and increased marginal zone B cells. These three pathways are feasible mechanisms for MeHg-induced autoimmunity. In addition, we identified candidate epigenetic biomarkers of PUFA-mediated protection: hypomethylated enhancer binding sites for retinoid X receptor (RXR) and retinoic acid receptor α (RARα). Last, we observed hypomethylated enhancer and promoter binding sites for glucocorticoid receptor (GR), which is associated with developmental neurotoxicity, and transcription factor 7-like 2 (TCF7L2), which is associated with type 2 diabetes (T2D) risk. Discussion: Here, we identify a set of candidate epigenetic biomarkers for assessing individualized risk of autoimmune response and protection against neurotoxicity due to MeHg exposure and fish consumption. In addition, our results may inform surrogate tissue biomarkers of early MeHg exposure-related neurotoxicity and T2D risk.

The exposome depicts the total exposures in the lifetime of an organism. Human exposome comprises exposures from environmental and humanistic sources. Biological, chemical, and physical environmental exposures pose potential health threats, especially to susceptible populations. Although still in its nascent stage, we are beginning to recognize the vast and dynamic nature of the exposome. In this review, we systematically summarize the biological and chemical environmental exposomes in three broad environmental matrices—air, soil, and water; each contains several distinct subcategories, along with a brief introduction to the physical exposome. Disease‐related environmental exposures are highlighted, and humans are also a major source of disease‐related biological exposures. We further discuss the interactions between biological, chemical, and physical exposomes. Finally, we propose a list of outstanding challenges under the exposome research framework that need to be addressed to move the field forward. Taken together, we present a detailed landscape of environmental exposome to prime researchers to join this exciting new field.

Swiss Cohort & Biobank – The White Paper

A large Swiss Cohort with 100,000+ representative participants of all ages is the backbone of public health related research and an essential point of reference for health systems steering, personalized health, and clinical research. The national multidisciplinary public health sciences community of SSPH+ and the national society of public health professionals-the Swiss Society for Public Health-assembled an ad hoc Steering & Writing Committee led by Prof. Nicole Probst-Hensch, Swiss TPH to publish a White Paper in fall 2022. This is a further complementary step in the national move toward better health data in Switzerland and specifically toward a large population-based cohort and biobank, with its many milestones, including the cohort pilot study of the Federal Office of Public Health (FOPH), largely based on the protocols of the only national population-based cohort and biobank of Switzerland (SAPALDIA) running successfully since >30 years as well as draft questionnaires of HBM4EU, Food frequency questionnaires of MenuCH, worker exposure questionnaires, and others. Some protocols have been developed specifically the cohort pilot, e.g., all blood and urine sampling protocols. Moreover, the parliamentary petitions (Motion 19.4069 1 ; Postulat 21.3220 2 ) call for a Children Cohort as another crucial step in the promotion of more Swiss research for the health of the youngest.
