Objective To report the incidence of trigeminal neuropathy seen among new patients in a referral center within a period of 1 year (2013). The cause of damage, method of management and treatment outcome was assessed after 1-year follow-up. Materials and Methods The records of all new patients visiting the oral and maxillofacial unit of the University hospital of Leuven in 2013 were screened for a history of damage to branches of the trigeminal nerve. The selected records were examined and the duration of nerve damage, received treatment as well as the outcome of the neuropathy after treatment was noted after 1-year follow-up. Results 56 patients (21 males, 35 females) from 7602 new patients had symptoms of damage to the trigeminal nerve branch. These symptoms persist in more than onethird of the patients [21/56 (37.5 %)] after 1-year followup. The least recovery is seen from oral surgery, implant placement, orthognathic surgery and tooth extraction. After 1 year 85 % (12/14) of neuropathic pain cases still have their symptoms as compared to 19 % (5/26) of patients with hypoesthesia. Conclusion This study shows a low incidence of nerve damage among the new patients presenting in oral and maxillofacial surgery clinic (\1 %); however, one-third of patients who sustain nerve damage never recover fully. Early diagnosis of the cause of neuropathy is essential. There is a need to objectively assess all patients with symptoms of trigeminal nerve damage before, during and after treatment.

ÖZDiş hekimliğinde periferik sinir yaralanmaları, rutin işlemler sırasında, cerrahi uygulamalarda veya travma sonucunda sıklıkla karşılaşılan olgulardır. Sinirin yapısı ve sinir hasarı mekanizmaları konusunda bilgi sahibi olmak, doğru teşhis ve tedavi için oldukça önemlidir. Sinir yaralanmalarının tedavisi, temel olarak medikal tedavi ve cerrahi tedavi olarak ikiye ayrılır. Medikal tedavinin sonuç vermediği hastalarda, muhtemel cerrahi tedavi için seçenek ve sonuçlarını bilmek, bu kritik komplikasyonda, doğru zamanlama ve uygulama açısından büyük önem arz etmektedir. Bu derlemenin amacı; diş hekimliği klinik uygulamalarında karşılaşıla-bilecek periferik sinir yaralanmaları sebeplerini hatırlamak ve mevcut kitap bilgileri ile güncel literatür bilgilerini harmanlayarak bu önemli komplikasyon hakkında bilgi sahibi olmaktır. Anahtar kelimeler: diş hekimliği; medikal tedavi; periferik sinir yaralanması; sinir cerrahisi GĠRĠġSinir sistemi iç ve dış ortamda oluşan değişik-liklere ani yanıtın oluşturulduğu sistemdir. Management of nerve injury is basically divided in two as medical and surgical treatment. It has a great importance to be aware of surgical treatment options and their possible results for correct timing and treatment in patients who do not respond to medical therapy. The purpose of this review is to to have information about peripheral nerve injuries and recent treatment approaches in clinical dental practice combining the book information and current literature. Hemen altındaki perinöriyum, fasikül adı verilen akson demetlerini saran kuvvetli bir bağ dokusu olup, travma ve iskemiye karşı çok dayanıklıdır ve bu bariyerin bozulması halinde sinir iletimi önemli ölçüde etkilenir. Endonöriyum ise sinir liflerini çevreleyen gevşek bağ dokusu olup, içerisinde aksonlar, Schwann hücreleri, kollajen lifler, fibroblastlar, kapiller damarlar ve az sayıda mast hücresi ile makrofajlar vardır. Paraesthesia of the mental nerve can occur due to various etiological factors. Rarely, dental infections can cause paraesthesia. However, this article discusses two cases of endodontic etiology in the mental nerve region as a causative factor for paraesthesia. In the first case, the patient had severe pain localized to his right mandible, with numbness of his lower lip. Endodontic treatment led to quick regression and resolution of paraesthesia. In the second case, a patient who was referred for retreatment of a mandibular second premolar infection developed profound paraesthesia in the region of the mental nerve distribution following prior therapy. Possible mechanisms responsible for periapical infection-related paraesthesia are discussed here. CBCT imaging may be useful in the diagnosis and management of such conditions.
