The subclass of triglyceride-rich lipoproteins -remnant-like particles (RLP) seems to be strong and independent risk factor for cardiovascular disease. We evaluated the role of RLP and other risk factors (RF) with sonographically measured intimamedia thickness of carotid arteries (IMT CCA) in a cohort of Czech population including women defined according to the time after menopause. We investigated relation of IMT CCA to age, weight, central obesity, plasma lipids including remnant-like particles cholesterol (RLP-C) and triglycerides (RLP-TG) in 136 men and 160 women. Using multiple linear regression analysis, significant association between IMT CCA and RLP-C was found in women 1-7 years after menopause. In the whole group of women, only age and fasting blood glucose were independently associated with IMT CCA. In men only age significantly correlated with IMT CCA. Significant decrease of all plasma lipids between 1988 and 1996 in men was detected, while in women significant increase in triglycerides and no change in non-HDL cholesterol was observed. RLP-C was the strongest independent RF for atherosclerosis in postmenopausal women but its association with IMT CCA was limited to several years after menopause. In conclusion, women changing reproductive status could be more sensitive to atherogenic impact of remnant lipoproteins.

Úvod: Ateroskleróza je hlavní příčinou mortality v České republice. V našich předchozích průřezových studiích jsme nalezli vysokou prevalenci metabolických kardiovaskulárních rizikových faktorů u pre-a postmenopauzálních žen a zároveň jsme zjistili, že přechod do menopauzy představuje kritické období, kdy dochází ke zrychlení progrese aterosklerózy. V této longitudinální studii jsme se zabývali změnami hlavních kardiovaskulárních rizikových faktorů u žen po přechodu do menopauzy. Metody: Analyzovali jsme údaje 195 žen, které v období šesti let dospěly do menopauzy, a 292 žen, které ve stejném časovém úseku zůstaly v menopauze. Mezi sledované kardiovaskulární rizikové faktory patřily kuřáctví, body mass index, obvod pasu, krevní tlak, plazmatické lipidy včetně hodnot apolipoproteinu B a A-1 a glykemie nalačno. Výsledky: Nejnápadnější odlišnosti mezi nově menopauzálními ženami a ženami trvale v menopauze byly zjištěny ve změnách plazmatických lipidů. S výjimkou HDL cholesterolu byly všechny změny u nově menopauzálních žen nepříznivé a nebyly spojeny s léčbou statiny. Mezi oběma skupinami nebyly pozorovány žádné statisticky významné rozdíly ve změnách body mass indexu, obvodu pasu, krevního tlaku nebo glykemie nalačno. Závěry: V longitudinální studii jsme potvrdili, že přechod do menopauzy je jedním z nejdynamičtějších období z hlediska změn kardiovaskulárních rizikových faktorů, hlavně plazmatických lipidů.

The aim of this study was to determine whether there is an association between leukocyte telomere length (LTL), and estrogen level, oxidative stress, cardiovascular disease (CVD) risk factors, and cardiorespiratory fitness (CRF) in pre- and postmenopausal obese women. Fifty-four obese women (premenopausal, n=25; postmenopausal, n=29) were selected to participate in this study. The outcome measurements in the pre- and postmenopausal groups were compared using independent t-tests and Pearson correlation analysis. The estrogen level (P<0.001), LTL (P<0.05), high-density lipoprotein level (P<0.05), and CRF (P<0.001) were higher in premenopausal women than in postmenopausal women. The body fat percentage (P<0.05) and triglyceride concentration (P<0.05) were lower in premenopausal women than in postmenopausal women. There were no significant associations between LTL, CVD risk, CRF, and oxidative stress and antioxidant enzyme activity in pre-menopausal women. The body mass index (BMI) and body fat percent-age in postmenopausal women were negatively associated with LTL (P<0.05). When all women were considered (i.e., both pre- and post-menopause), the BMI, percentage of fat, and waist circumference had a negative association with LTL (P<0.05), and estrogen levels were positively associated with LTL (P<0.05). Decreased estrogen levels after menopause, a pivotal factor in the biology of aging, and obesity were more associated with shorter telomere lengths in pre- and postmenopausal women than aerobic capacity and other CVD risk factors.
