Meningococcal purpura fulminans in children. II: Late orthopedic sequelae management

Abstract: It is important for children diagnosed with meningococcal purpura fulminans to be followed up closely starting from the very beginning by a pediatric orthopedic surgeon. It ensures that late-onset orthopedic sequelae will be diagnosed early. In accordance to the literature, this study highlights the high rate of lower limb epiphysiodesis, above all other types of sequelae. This study reports a possible link between purpura fulminans and avascular necrosis of the hip.

“…The role of epiphysiodesis in preventing recurrent deformity has also recently been highlighted by Nectoux et al 9,10 Varus deformity around the knee as a result of meningococcal infection is similar to that caused by Blount's disease, with a variable extent of medial tibial plateau involvement. Hemiplateau elevation and bone graft may be necessary to restore joint congruity.…”

“…Ранние орто педические последствия наблюдаются на фоне нарушения кровообращения в виде тяжелых некробиотических изменений мягких тканей как проксимальных, так и дистальных отделов конечностей. При отторжении некротических масс формируются грубые рубцы мягких тканей, а при наличии глубоких дефектов кожи и мягких тканей может понадобиться некрэктомия с последующим пластическим закрытием кожного дефекта или ампутация дистальных сегментов конечностей [6,10].…”

“…Soft tissue and bone infections at the distal portion of an amputation stump occurred in 17% at the mean of 7 years after the initial sepsis. Nectoux et al 12 reported a total of six stump revisions because of soft tissue coverage or to improve prosthetic wear in seven patients who had below knee amputations.…”

“…Nectoux et al 12 reported the outcome of 14/19 patients who developed at least 1 orthopaedic consequence after a mean of 2 years. The most common presentation was lower limb growth plate arrest in 8 patients involving 18 growth plates leading to limb length discrepancy in 5 patients and significant knee or ankle malalignment in 12 patients.…”
