Meningitis and endocarditis as a sequela of streptococcus pneumonia mastoiditis: A case report

Mitchell Peebles,
Mehnaz Roshani,
Kumaraman Srivastava

Abstract: Austrian Syndrome classically consists of meningitis, endocarditis, and pneumonia due to Streptococcus pneumonia bacteremia. A literature review, however, does not show variants of this triad. Our case highlights a unique variant of Austrian Syndrome with mastoiditis, meningitis, and endocarditis which requires immediate recognition and treatment to prevent devastating patient outcomes.

“…Acute bacterial sinusitis is common in children, and despite being mostly a disorder with a favorable course, a percentage ranging from 3.7% to 8.8% may present serious complications. 2 Furthermore, as outlined by Smiljkovic et al, 1 there is a risk of persistent neurological sequelae, especially in children with extensive intracranial involvement. We retrospectively analyzed data of 38 children admitted to our Hospital from January 2013 to December 2023 due to complicated sinusitis.…”

“…1 Neurological complications are frequent in IE, occurring in 20-40% of cases. Although meningitis has been reported in approximately 7% of IE patients, 2,3 the link between meningitis and IE has not received much attention, particularly when meningitis is the presenting symptom. In many instances, meningitis may present as an early indication of IE, with over a third of patients who experience meningitis going on to develop further neurological complications, such as ischemic stroke, intracranial hemorrhage, or brain abscess.…”
