Frequencies of CT and MRI findings characteristic of meningiomas were compared in 50 cases. Plain and contrast enhanced examinations with CT and MRI were evaluated retrospectively regarding 12 criteria known to be indicative of the diagnosis of meningiomas. CT proved to be superior in demonstrating calcifications and a typical tumor density. On the other hand. MRI was better suited for identifying the extraaxial location of tumors, the broad contact of tumors to the meninges, tumor capsules and meningeal contrast enhancement adjacent to the tumor. Both methods provided nearly equal results in demonstrating mass effects, hyperostoses, intensive and homogeneous contrast enhancement, and smooth tumor contours after contrast administration. On the whole, neither of the two methods demonstrated a universal superiority for the diagnosis of intracranial meningiomas. Rather, each method displayed distinct advantages.

The purpose of this study is to evaluate the incidence and the degree of peritumoral edema on MRI in meningioma and to correlate other MR findings withtheedema. Materals and Methodds: MR images of 35 patients with histologically-confirmed meningioma were retrospectively reviewed. We analyzed the MR findings with special attention to the presence or absence and degree of edema. The edema was graded as absent, mild (extending less than 1 cm from outer margin of mass), moderate (1 to 3cm with mild mass effect), and severe (more than 3cm with marked mass effect). We also evaluated size and margin of the tumor, heterogeneity of mass signal , enhancement pattern and dural enhancement of the masses. In 24 patients with cerebral angiography, cerebral vascularity on angiogram was correlated with MR findings. Statistic correlation analysis was done using SAS ver 6.04. Results: Twenty-five of 35 cases (72%) had edema; mild in 11 cases , moderate in 10 cases, and severe in 4 cases. Heterogeneous signal intensity of mass (< 0. 001), heterogeneous enhancement of mass (< 0.001), mass size (< 0.001), 10cation (< 0.001), and vascularity o'n angiography (< 0.05) were well correlated with edema, while mass margin (> 0.05), dural enhancement (> 0.05), and histologictype (> 0.05) were not correlated with edema. Conclusion : In meningioma, moderate to severe peritumoral edema occurred in 41 % (14/ 35). The edema was correlated with heterogeneous enhancement, size, location , heterogeneous signal intensity and vascularity of the mass on angiography .

RIASSUNTO -Il seno cavernoso puo essere interessato da patologia neoplastica (primitiva e secondaria), infiammatoria e vascolare. Tutte queste entita, ad esclusione delle fistole carotido-cavernose dirette, si manifestano con una clinica analoga, rendendo la diagnosi dipendente fondamentalmente dalle tecniche di imaging. Attualmente le metodiche piu utili nello studio del seno cavernoso sono la tomografia computerizzata e la risonanza magnetica che, con le tecniche di angio-TC ed angio-RM, permettono anche uno studio simil-angiografico della regione di interesse. L'arteriografia selettiva rimane tuttora la tecnica gold standard nello studio delle patologie vascolari (aneurismi della carotide interna e fistole carotido-cavernose ). La flebografia sovraorbitaria trova attualmente utilizzo esclusivo nella diagnosi della sindrome di Tolosa Hunt con RM negativa e talvolta nella terapia endovascolare di fistole artero-venose.I tumori primitivi piu frequenti sono i meningiomi, mentre i neurinomi del seno cavernoso sono estremamente rari. I meningiomi insorgono dal rivestimento durale del seno e si manifestano, sia alla TC che alla RM, come lesioni ben delimitate con un'impregnazione precoce, intensa ed omogenea. I neurinomi del seno cavernoso possono derivare dal Ill, dal IV, dal V o dal VI nervo cranico e possono insorgere primitivamente nel seno cavernoso o, piu frequentemente, negli spazi cisternali interessando il seno sviluppandosi lungo il nervo. La diagnosi differenziale deve essere posta soprattutto tra il meningioma ed il macroadenoma ipofisario a sviluppo laterosellare (il piu frequente tumore secondario ). 11 principale criterio riguarda l'interessamento della carotide interna che viene frequentemente stenotizzata dai meningiomi, mentre puo essere circondata e dislocata dai macroadenomi, senza pero apprezzare significative riduzioni del suo lume. Il seno cavernoso puo essere interessato per contiguita anche da due neoplasie della base cranica: il carcinoma del rinofaringe ed il cordoma.Le metastasi del seno cavernoso si possono instaurare per via ematogena, liquorale o perineurale. Determinano generalmente aumento di volume del seno, non hanno un segnale RM caratteristico e devono essere messe in diagnosi differenziale con le patologie infiammatorie. Queste comprendono la sindrome di Tolosa Hunt e le affezioni granulomatose croniche (sarcoidosi, granulomatosi di Wegener) . La prima e caratterizzata da dolore retroorbitario, paralisi dell'oculomotore ed iperestesia trigeminale sostenute da un'infiammazione del seno ad eziologia sconosciuta. Le immagini di RM possono essere del tutto negative ed in tali casi puo essere utile per raggiungere la diagnosi la flebografia. Un valido criterio diagnostico per le patologie infiammatorie e rappresentato dalla drammatica remissione della sintomatologia e del quadro radiologico in seguito a terapia steroidea.Gli aneurismi della carotide interna sono classificati in base alle dimensioni: se di diametro superiore ai 2,5 cm vengono definiti giganti.
