Membranoproliferative glomerulonephritis in a patient with chronic brucellosis

Abstract: Brucellosis is the most common zoonotic disease in Greece, with an endemic distribution and can affect any organ. Infiltration of the renal parenchyma causes acute and chronic interstitial nephritis with granulomas, whereas renal glomeruli are rarely affected. The disease has been sporadically reported, and it causes various histopathologic patterns. Herein, we describe the case of a 39-year-old stock breeder with a history of recurrent episodes of bacteremia caused by Brucella melitensis over a period of 3 ye… Show more

“…Cryoglobulinemia and hypocomplementemia but no ANCA were present. Renal biopsy revealed MPGN and he was partially remitted under antimicrobial and corticosteroid therapy [ 27 ]. In such circumstances, renal biopsy was obligatory to clarify the diagnosis, guide the treatment and predict the prognosis.…”

“…The application of corticosteroids improved the prognosis of renal injury associated with brucellosis. The corticosteroid was applied when the specific antimicrobial agent didn’t work or when the patient was accompanied by rapid progressive renal insufficiency, nephrotic proteinuria, aortitis or accompanied with vasculitis [ 27 , 35 – 37 ]. The patient usually had a good response to corticosteroids.…”

“…Brucella invasion is the first step in which phagocytes play a role; the bacteria first adhere to host cell membrane through lipopolysaccharide receptors and lipid valves. FC receptors play a role in processes such as intrusion of bacteria into the cell to reproduce, transfer through blood, and lymph node invasion of host organs through adhesion, host response, organ inflammation, and organ lesion ( Amjadi et al, 2019 ), such as membranoproliferative glomerulonephritis ( Provatopoulou et al, 2018 ), testicular abscess ( Vallianou et al, 2018 ), and splenic abscess ( Sudulagunta et al, 2017 ).…”
