Melasma update

Abstract: Melasma is an acquired pigmentary disorder characterized by symmetrical hyperpigmented macules on the face. Its pathogenesis is complex and involves the interplay of various factors such as genetic predisposition, ultraviolet radiation, hormonal factors, and drugs. An insight into the pathogenesis is important to devise treatment modalities that accurately target the disease process and prevent relapses. Hydroquinone remains the gold standard of treatment though many newer drugs, especially plant extracts, hav… Show more

“…Patients with melisma were treated with topical silymarin cream 0.1% and 0.2%, two times per day for 4 weeks, and no adverse effects were reported (Sarkar, Arora, Garg, Sonthalia, & Gokhale, 2014). In a randomized, double-blinded placebo controlled trial on patients with gastrointestinal cancer who had hand-foot syndrome due to capecitabine, application of silymarin gel 1% twice daily for 9 weeks had no adverse effects (Elyasi et al, 2017).…”

“…Its treatment often results in unsatisfactory outcomes and hence it remains a challenge (1). Melasma affects about 1.5% to 33.3% of people (2). The main pathogenesis of melasma is not completely known (3) although vascularization is considered a source of development (4).…”

“…Vascularization is caused by angiogenesis (creation of new vessels) as a consequence of regulators such as vascular endothelial growth factor (VGEF). High vascular endothelial growth factor could be a possible reason for the increase of vascularization in melasma lesions (2).…”

“…Depigmenting drugs, retinoids, and sunscreen are the mainstay of therapy for extrafacial melasma. [ 5 6 ] It is premature to comment whether extrafacial melasma responds to depigmenting therapy akin to typical facial melasma.…”
