Melanom-assoziierte Retinopathie (MAR) mit Nachtblindheit

Abstract: Results indicate the presence of melanoma-associated retinopathy (MAR), which-like carcinoma-associated retinopathy (CAR)-ranks among the tumor-associated diseases of the retina. CAR, MAR, and CSNB can be differentiated immunohistochemically by serum autoantibody determination and electrophysiologically by flash-ERG. As opposed to CAR, the immune response in the case of MAR is not to antigens of photoreceptors and ganglion cells, but to retinal so-called ON-depolarizing bipolar cells mainly connected in series… Show more

“…In contrast to the results published by Klopfer et al [12]the antibody reaction to the bipolar cell layer was not typical. Sudden onset and irreversibility of symptoms as well as electrophysiological and psychophysiological findings are the major features that led us to the diagnosis MAR in patient 1.…”

“…Several studies discuss direct toxic damage to the retinal cells by a circulating product of the tumor cells itself. Other investigators hypothesize that there are autoantibodies against a tumor-specific epitope that cross-reacts with a retinal antigen [1, 11, 12, 13]. Milam et al [10]postulated in their immunofluorescence studies that MAR patients generate autoantibodies against a melanoma antigen that interferes with bipolar cells of the retina, which is reflected by a decreased amplitude of b-wave in ERG.…”

“…In terms of CAR, a better prognosis for patients has also been pointed out [15,16]. Indeed, the CAR + lung small cell carcinoma patient 2 in our study has a clinically good prognosis.…”

“…Recoverin is also one of the isolated antigens such as carcino-testicular proteins or carcinoneural protein [14]. Interestingly, as in some paraneoplastic syndromes, it has been suggested that CAR + cancer patients possessed a better prognosis than CAR -cancer patients [15,16]. Thus, we hypothesize that recoverin-specific CTL can be efficiently induced, and that recoverin-expressing cancer cells can be killed by these CTL in CAR + cancer patients.…”

“…A recent review noted that all reported CAR cases had a very limited life span, and this precluded the observation of any long-term effects of treatment on visual function [16]. Conversely, it is has been suggested that patients with CAR had a better prognosis than those without CAR [17,18]. Maeda et al referred to the favorable prognosis of patients with CAR by describing that one of two SCLC patients with CAR in their study continued to lead an ordinary life for several years at that time [10].…”
