In pharmacies in Sweden, a clinical decision support system called Electronic Expert Support (EES) is available to analyse patients’ prescriptions for potential drug-related problems. A nationwide intervention was performed in 2018 among all Swedish pharmacy chains to increase the use of EES among patients 75 years or older. The aim of this research was to study the use of EES in connection with the national intervention in order to describe any effects of the intervention, to understand how pharmacists use EES and to identify any barriers and facilitators for the use of EES by pharmacists for elderly patients. Data on the number and categories of EES analyses, alerts, resolved alerts and active pharmacies was provided by the Swedish eHealth Agency. The effects of the intervention were analysed using interrupted time series regression. A web-based questionnaire comprising 20 questions was sent to 1500 pharmacists randomly selected from all pharmacies in Sweden. The study shows that pharmacists use and appreciate EES and that the national intervention had a clear effect during the week of the intervention and seems to have contributed to a faster increase in pharmacists’ use of EES during the year to follow. The study also identified several issues or barriers for using EES.

The aim of this study was to evaluate the provision of a professional pharmaceutical patient-centered model in a weight management program and optimization of the medication in a Spanish community pharmacy. Patients and Methods: This was a single-group intervention study with a mean follow-up period of 8.2 months (sd 2.3). Patients ≥18 years old seeking to lose weight or improve eating habits were recruited. On the first visit, the pharmacist collected patients' sociodemographic and anthropometric variables, dietary history and lifestyle habits, biochemical measurements and other clinical and therapeutic data. The intervention was based on the Spanish Society of Community Pharmacy recommendations for diet and exercise and for pharmacotherapy management. The follow-up included a two-month visit and a final visit. Results: A total of 330 patients were included (80% women; mean age 51.3 years old (sd 15.3)). A statistically significant reduction in anthropometric measurements (weight, BMI, and waist circumference) and a statistically significant increase in the number of patients with normal cholesterol and LDL-cholesterol (p<0.001) were observed at two-month visit compared with first visit (p<0.001). The number of patients with normal triglyceride levels at final visit compared with first visit also increased significantly (p=0.04). A total of 186 (56.4%) patients had drugrelated problems at first visit and 31 (9.4%) patients at two-month visit. Conclusion: The implementation of a patient-centered weight management model had a positive impact on the improvement of anthropometric, clinical and therapeutic parameters.

Zusammenfassung Einleitung Der Gesamtüberblick über die Medikation eines Patienten ist wesentliche Voraussetzung für die Arzneimitteltherapiesicherheit. Ein aktueller und vollständiger Medikationsplan (MP) ist hierfür ein geeignetes Instrument. Ziel war zu untersuchen, inwiefern die elektronische Bearbeitung des bundeseinheitlichen Medikationsplans gemeinsam durch Arzt und Apotheker in ihren Primärsystemen technisch machbar ist, die definierten Prozesse praktikabel sind und akzeptiert werden. Methoden Ärzte und Apotheker testeten in einem mehrstufigen Verfahren die elektronische Umsetzung. Die Bearbeitung der MP erfolgte anhand definierter Prozesse und Verantwortlichkeiten; Machbarkeit und Akzeptanz wurden in schriftlichen Befragungen und einem Workshop evaluiert. Ergebnisse Erstmalig in Deutschland wurden die Voraussetzungen zur elektronischen Erstellung, zum Austausch und zur Aktualisierung von MP in einem Arzt- und vier Apothekenverwaltungssystemen geschaffen. Gelöste Probleme waren technische und AMTS-relevante Fehler, abweichende Darstellungen, mangelnde Benutzerfreundlichkeit und begrenzte Speicherkapazität des Barcodes. Elf Arzt-Apotheker-Teams betreuten 196 Patienten. Insgesamt waren 60 % mit den Prozessen und 80 % der Ärzte sowie 63 % der Apotheker mit den Verantwortlichkeiten zufrieden. Ärzte bewerteten die Erfassung der Gesamtmedikation durch Apotheker positiv. Die Kooperation verbesserte den fachlichen Austausch. Dies führt bei 70 % der Ärzte bei der Medikation und bei 88 % der Apotheker zum Gesundheitszustand der Patienten zu einem Wissenszuwachs. Alle Ärzte und Apotheker sahen eine strukturierte Zusammenarbeit als wichtig (25 %) oder sehr wichtig (75 %) für die Qualität der MP an. Diskussion Der MP wurde erstmalig in Primärsysteme implementiert. Prozesse und Verantwortlichkeiten fanden bei Ärzten und Apothekern Akzeptanz. Dies sind wichtige Voraussetzungen, um den MP flächendeckend und nachhaltig zu implementieren.
