Medication Adherence After Renal Transplantation—a Review of the Literature

Abstract: Reasons for non-adherence after kidney transplantations are diverse. Attention is attracted by the fact that potentially modifiable factors such as social support, experiences on dialysis, side effects, features of the treatment regimen, intentions and beliefs, forgetfulness and mental health issues play a greater role than other factors in the development of medication non-adherence. Factors not related to patient characteristics seem to be under researched.

“…16 Factors that underlying their decisions vary from experiencing adverse effects 17 to their perceptions of the need for the prescribed treatment. 18 The concept of intentionality has been used to examine treatment nonadherence among individuals living with a variety of chronic conditions, including coronary heart disease, [19][20][21] glaucoma, 22 kidney disease, 23 hypertension, 20,24 epilepsy, 25 and diabetes. 21,24 This research has led to tailored interventions, with mixed results, and recommendations that focus on intentional and unintentional nonadherence.…”

“…To date, little is known about haemodialysis patients’ perceptions regarding their prescribed regimen and the factors influencing their medication-taking behaviour. Understanding patients’ perspectives can help identify potentially modifiable factors such as patients’ intention to adhere, beliefs about medicines, features about treatment regimens, experiences of side effects, and provision of support mechanism required to facilitate adherence [17]. As such, we aimed to qualitatively explore factors associated with medication adherence, and examine the differential perspectives on medication-taking behaviour shown by haemodialysis patients.…”

“…Reasons for non-adherence vary between age groups and include side-effect profiles, perceived harm, changes to physical appearance and psychological distress. At all clinic visits, non-adherence should be considered and discussed with patients and their family, ideally by the specialist pharmacist 23…”
