BackgroundEmergency departments (EDs) offer a variety of learning opportunities for undergraduate medical students. It is however, difficult to evaluate whether they are receiving recommended training during their emergency medicine (EM) clerkship without identifying their clinical activities. We aimed to evaluate the clinical exposure of the final year medical students at our College during their EM clerkship.MethodsThis is a retrospective analysis of prospectively collected student logbooks. 75 students rotated in a 4-week EM clerkship during 2015–2016. The students rotated in EDs of two hospitals. Each ED treats more than 120,000 cases annually. The students completed 12 eight-hours shifts. Presentations and procedures seen were compared with EM curriculum recommendations.ResultsFive thousand one hundred twenty-two patient presentations and 3246 procedures were recorded in the logbooks, an average (SD) of 68.3 (17.6) patients and 46.1 (14.0) procedures. None of the students encountered all ten recommended presentations. Two students (2.6%) logged all nine procedure categories of the EM curriculum.ConclusionRecommended presentations and procedures of the EM clerkship were not fully encountered by all our students. Different settings vary in the availability and type of patients and procedures. Cinsel saldırı mağdurlarının sosyodemografik özellikleri, saldırı çeşidi, adli muayene bulguları ve aralarındaki ilişkinin değerlendirilmesidir. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Çalışmada, Etlik Zübeyde Hanım Sağlık Uygulama Araştırma Merkezi Acil Servisi'ne 1 Ocak 2014-30 Kasım 2017 tarihleri arasında adli makamlarca cinsel saldırı şüphesiyle gönderilen 18 yaş ve üzerindeki bireylerin geriye dönük bir şekilde incelenmesi esas alınmıştır. Hasta dosyaları ve adli raporları araştırmacılar tarafından tasarlanan bir form aracılığıyla değerlendirilmiştir. B Bu ul lg gu ul la ar r: : Çalışma kapsamında bildirilen 92 bireyden, araştırmacılar tarafından belirlenen kriterleri sağlayan 80 olgu çalışmaya dâhil edilmiştir. Bildirilen bireylerin tamamının kadın olduğu, %31,3'ünün ise bekâr olduğu saptanmıştır. Minimum yaşın 18, maksimum yaşın 68 ve ortalama yaşın 28,21±10,15 yıl olduğu belirlenmiştir. Cinsel saldırıların %56,2'sinin failin evinde veya aracında gerçekleştiği, bireylerin %32,5'inin saldırıdan 48 saat sonra bildirim yaptığı görülmüştür. Bireylerin %70'inde failin tanıdık, %10'unda failin birden fazla olduğu saptanmıştır. Mağdurların %85'i ile muayene öncesinde psikolojik ilkyardım görüşmesinin yapıldığı belirlenmiştir. Bu çalışmanın bulguları, evli mağdurların daha fazla tekrarlayan saldırıya, bekâr mağdurların ise daha yüksek düzeyde penetrasyon bulgusuna sahip olduğunu ortaya koymuştur. Yabancıya göre tanıdık saldırgan mağduru olmanın daha yüksek düzeyde tekrarlayan saldırı ile ilişkili olduğu saptanmıştır. Mağdurun yaşının cinsel saldırı çeşidi ve muayene bulgularıyla ilişkisiz olduğu belirlemiştir. S So on nu uç ç: : Cinsel saldırı vakalarının gözden kaçırılması, mağdura ihtiyacı olan yardımı sunamama, şiddetin tekrar etmesine hatta dozajının artmasına neden olabilmektedir. Acil servisler cinsel şiddeti belgelendirme ve önlemede kilit role sahip kurumlardan biridir. Bulgular, cinsel saldırılarda faillerin sıklıkla mağdurların tanıdığı kişiler olduğunu, evli mağdurların bekârlara göre daha yüksek oranda tekrarlayan saldırıya maruz kaldığını ortaya koyması açısından önemlidir. Adli muayene işleminin multidisipliner yaklaşımla ele alınması, delillerin sağlıklı bir şekilde toplanmasının yanı sıra muayene sonrası mağdurun yaşamını planlamasına yardımcı olacaktır.

Objectives:The goal of this study was to develop and evaluate a novel curriculum and assessment tool for Core Entrustable Professional Activity (EPA) 10 competencies and entrustment scoring in a cohort of medical students in their emergency medicine (EM) clerkship using a framework of individualized, ad hoc, formative assessment.Core EPA 10 is an observable workplace-based activity for graduating medical students to recognize a patient requiring urgent or emergent care and initiate evaluation and management.Methods: This is a prospective, pretest-posttest study of medical students during their EM clerkship. Using the Thomas and Kern framework, we created a curriculum of simulation cases about chest pain/cardiac arrest and respiratory distress, which included novel assessment checklists, and instructional videos about recognizing and managing emergencies. Students were individually pretested on EPA 10 competencies using the simulation cases. Two raters scored students using standardized checklists. Students then watched instructional videos, underwent a posttest with the simulation cases, and were scored again by the two raters using the checklists.Differences between pretest and posttest scores were analyzed using paired t-tests and Wilcoxon signed-rank tests.Results: Seventy-three out of 85 (86%) students completed the curriculum. Mean scores from pretest to final posttest in the chest pain/cardiac arrest and respiratory distress cases significantly improved from 14.8/19 (SD 1.91), to 17.1/19 (SD = 1.00), t(68) = 10.56, p < 0.001, and 8.5/13 (SD 1.79), to 11.1/13(SD 0.89), t(67) = 11.15, p < 0.001, respectively. The kappa coefficients were 0.909 (n = 2698, p < 0.001) and 0.931 (n = 1872, p < 0.001). Median modified Chen entrustment scores improved from 1b (i.e., "Watch me do this") to 2b (i.e., "I'll watch you") for the chest pain/cardiac arrest case (p < 0.001) and 1b/2a (i.e., "Watch me do this"/ "Let's do this together") to 3a (i.e. "You go ahead, and I'll double-check all of your findings") for the respiratory distress case (p < 0.001). Conclusion:A new directed curriculum of standardized simulation cases and asynchronous instructional videos improved medical student performance in EPA 10
