Purpose Prior to the COVID-19 pandemic, telemedicine in orthopaedics and trauma surgery had mostly developed for joint arthroplasty, fracture management, and general pre- and postoperative care including teleradiology. With the corona-outbreak, telemedicine was applied on a broad scale to prevent assemblage and to guarantee access to medical care protecting critical areas. The purpose of the present study was to give an overview of the spectrum of clinical applications and the efficacy of telemedicine in orthopaedic and trauma surgery as published in times of the COVID-19 pandemic. Methods All published studies investigating the application of telemedicine related to orthopaedics and trauma during the COVID-19 pandemic were accessed and screened for suitability. The primary outcome of interest was the efficacy of telemedicine in various clinical applications. The secondary outcome of interest was the spectrum of different applications in which telemedicine applications were investigated. Results The literature search resulted in 1047 articles. After the removal of duplicates, 894 articles were screened of which 31 finally met the inclusion criteria. Dimensions that were described by studies in the literature to have positive effects were preoperative patient optimisation, the usefulness of telemedicine to correctly diagnose a condition, conservative treatment, willingness to and feasibility for telemedicine in patients and doctors, and postoperative/post-trauma care improvement. The efficacy of telemedicine applications or interventions thereby strongly varied and seemed to depend on the exact study design and the research question addressed. Conclusion Various successful applications of telemedicine have already been reported in orthopaedics and trauma surgery, with a strong increase in scientific output during the COVID-19 years 2020–2021. Whether the advantages of such an approach will lead to a relevant implementation of telemedicine in everyday clinical practice should be monitored after the COVID-19 pandemic.

People in the epicenter suffer from emergency medical supplies shortage in the early stage of a public health emergency because of imbalanced supply-demand in different regions or areas, which is a key issue in a major infectious disease. In response to the severe insufficiency of supplies in the epicenter, this study proposed a strategy of distributing supplies from peripheral areas to the epicenter and gave a supply-side selection model considering the epidemic influence and supplies condition in the candidate supply-side areas. First of all, the epidemic spatial-temporal transmission path (STTP) network describing the geographic spread of disease is obtained using a first-order conditional dependence approximation algorithm in a dynamic Bayesian network (DBN). Then, the structural information of the STTP network and the supplies condition characteristic information are combined using the Bipartite network embedding (BiNE) method. Finally, a graph convolutional neural network (GCN) is conducted to select the supply-side areas for peripheral-epicenter supplies distribution based on information achieved from the bipartite graph. The results show that the highest supplies allocation accuracy reaches 87%. Validation and supremacy of the proposed methodology are provided by applying it to the case in Hubei province. This study considers crossed-areas supplies distribution strategy and contributes to select suitable supply-side areas considering the epidemic and supplies condition in the peripheral areas, which is helpful to both epicenter and peripheral areas.

Role and relevance of dentists in a multiprofessional palliative care team: results of a cross-sectional survey study

Purpose Despite the multiprofessional concept surrounding palliative care patients (PCPs) and their high prevalence of oral issues, licensed dentists (LDs) are often not included in their treatment team. This study aimed to examine the current state of cooperation and to determine whether and how LDs should be included in the care for PCPs. Methods This single-centre cross-sectional study was conducted at the University Hospital Muenster, Germany. We surveyed three participant groups: PCPs, LDs, and healthcare professionals (HCPs). Questionnaires were tailored for each group, with some questions common for comparison. Results The study encompassed the results of 48 questionnaires from LDs, 50 from PCPs along with 50 from HCPs. Consensus was reached among all parties (LDs: 73% (n = 35/48); HCPs: 94%, n = 47/50; PCPs: 60%, n = 30/50) that involving LDs in the treatment concept is favourable. On the other hand, a significant discrepancy emerged in the perception of the dental treatment effort required by PCPs. While LDs (81%; n = 39/48) and HCPs (64%; n = 32/50) were convinced of increased effort, PCPs (34%; n = 17/50) largely did not share this perspective. To enhance patient care and formulate appropriate treatment plans, LDs consider both training (58%; n = 28/48) and guidebooks (71%; n = 34/48) to be valuable and would attend or use such resources. Conclusion This study sheds light on the current gaps in including LDs in palliative care teams and emphasizes the importance of multidisciplinary collaboration to address oral health needs effectively. Development of continuing education options and collaborative models between LDs and HCPs needs to be further expanded in future.
