Mediation of the effect of childhood socioeconomic position by educational attainment on adult chronic disease in Chile

Abstract: Low ChSEP exerts a primarily direct effect on later chronic disease, modestly mediated by education. Education attainment showed larger direct effects with minimal mediation by behaviors. Strengthening current-early child development and education policies, particularly gender aspects, may reduce social inequalities and key pathways for reducing chronic disease inequalities in Chile.

“…Also, most studies have described these associations using only one of the described theoretical models (Reiss 2013;Frenz et al 2017). Indeed, to the best of our knowledge, there are only three studies that have tested these models simultaneously on health outcomes (Pudrovska and Anikputa 2014; Shuey and Willson 2014;Willson and Shuey 2016).…”

“…Despite the growing interest in assessing the long-term influence of CSES, the mediating mechanisms by which CSES affects chronic conditions remain unclear. Most previous studies assessed socioeconomic and behavioral factors in adulthood as mediators in the association between CSES and chronic conditions in adulthood (de Sousa Andrade et al 2017;Frenz et al 2017;Sheikh et al 2014). However, when assessing mediation one must assume a lack of intermediary confounders (Sheikh et al 2016b), i.e., that there are no variables (measured or unmeasured) affecting both the mediator(s) and the outcome that are affected by the exposure itself (Sheikh et al 2016b).…”

“…Furthermore, two values must be specified for the exposure: the base (a1) and alternative (a2) level. For the mediator, it is necessary to specify a value ‘m’ representing the level at which the controlled direct effect is to be estimated ( 37 ). Although the PE score, as well as control and demands, are available as continuous variables, we do not have any theoretical, statistical, or empirical criteria to define a priori values for (a1), (a2) and (m).…”
