Medial Sural Artery Perforator Flap: A Middle Ground Between Anterolateral Thigh and Radial Forearm Flaps

Abstract: ObjectiveThe medial sural artery perforator (MSAP) flap has gained popularity in head and neck reconstruction primarily as a less morbid alternative to the radial forearm free flap (RFFF). However, no direct comparison of thickness exists among the MSAP, RFFF, and anterolateral thigh (ALT) flaps, which together represent the commonly utilized nonosseus‐free flaps in head and neck reconstruction. Thus, this study aimed to compare these flap sites and identify predictors of thickness that will aid in reconstruct… Show more

“…While it is more commonly used for limb salvage and extremity reconstruction, the flap also represents a potential donor site when thinner tissue is needed for head and neck reconstruction ( Figure 6 and Figure 7 ). While the tissue may be somewhat thicker than an upper extremity flap, the tissue is still typically thinner and more pliable than the thigh [ 30 , 31 , 32 ]. One potential advantage of the MSAP over the upper extremity flaps is the opportunity for simultaneous harvest in conjunction with the resection [ 32 ].…”
