Mechanisms Underlying Conduction Slowing and Arrhythmogenesis in Nonischemic Dilated Cardiomyopathy

Abstract: Abstract-Heart Failure (HF) is associated with an increased risk of sudden death caused by ventricular tachyarrhythmias.Recent studies have implicated repolarization abnormalities and, in particular, exaggerated heterogeneity of transmural repolarization in the genesis of polymorphic ventricular tachycardia in a canine model of nonischemic dilated cardiomyopathy. The presence and degree to which conduction abnormalities play a role in arrhythmogenesis in this model are uncertain. HF was produced in dogs by rap… Show more

“…Recent work suggests an important role of connexin43 phosphorylation in HF (Akar et al, 2004;Ai and Pogwizd, 2005). We found greatly decreased phosphorylation of connexin43 in the stage of decompensated HF (21 weeks after operation).…”

“…We previously developed the technique of transmural optical action potential (AP) mapping in arterially perfused canine and porcine wedge preparations 13, 14, 15, 16. This technique has been extensively used by others in wedges from explanted human hearts 17, 18, 19, 20, 21, 22.…”

“…Briefly, wedges of porcine myocardium were dissected from the mid apico‐basal region adjacent to the anterior infarct, a region known to support post‐MI arrhythmias. Wedges were arterially perfused via a secondary branch of the high lateral or posterolateral left circumflex coronary artery and optical mapping was performed using the voltage‐sensitive dye, di‐4‐ANEPPS 13, 14, 15, 16, 23. Porcine wedge preparations were isolated from control (N=8 pigs, n=11 wedges), MI (N=5 pigs, n=10 wedges), and MI+SERCA2a (N=5 pigs, n=10 wedges) animals.…”

“…Tissue samples from the hearts of four unoperated normal dogs were similarly obtained. We have previously shown that sham operated and unoperated controls exhibit similar mechanical, neurohumoral and electrophysiological phenotypes, thus to reduce cost and morbidity for the animals unoperated controls were used [9,11,23,24]. The canine pacing tachycardia model is characterized by systolic LV failure, preserved RV systolic function and no significant inflammation or fibrosis [24].…”
