Mechanisms, contexts and points of contention: operationalizing realist-informed research for complex health interventions

Abstract: BackgroundThe concept of “mechanism” is central to realist approaches to research, yet research teams struggle to operationalize and apply the concept in empirical research. Our large, interdisciplinary research team has also experienced challenges in making the concept useful in our study of the implementation of models of integrated community-based primary health care (ICBPHC) in three international jurisdictions (Ontario and Quebec in Canada, and in New Zealand).MethodsIn this paper we summarize definitions… Show more

“…The articles considered decision making both in relation to whether to seek help from a service urgently and which service to then contact. It was often difficult to distinguish context and mechanism, because there were multiple mechanisms, some of which were often contexts for further mechanisms.…”

“…Realist review is a systematic, theory-driven approach that aims to make explicit the underlying processes, structures or reasoning (mechanisms) of how and why complex interventions work (or do not) in particular settings or contexts. 34,35 Namaste Care is a complex multicomponent intervention dependent on the behaviours and choices of those delivering and receiving the care. The purpose of this review was to develop an explanatory account or programme theory about Namaste Care and how it might work for people with advanced dementia living in long-term care settings, such as nursing homes.…”

“…Defining mechanisms and identifying them as a component separate to context is a widely recognised problem for realist researchers (Dalkin et al, 2015; Emmel et al, 2018; Lacouture et al, 2015; Marchal et al, 2012; Shaw et al, 2018). Our observation data focused on staff interactions with patients.…”
