Mechanism of granuloma formation in sarcoidosis

Abstract: We provide a comprehensive update on the cellular components and their functional implications in sarcoid granuloma formation, with special emphasis on the specific characteristics of granuloma in acute versus chronic sarcoidosis. Moreover, the specific antigens thought to be involved in both forms of the disease are discussed.

“…7 Granulomatous inflammation is associated with substantial pathology; in the extreme, it can arise in the absence of a detectable foreign stimulus, thus constituting the pathology by itself, as it is the case of sarcoidosis. 8 As attested by the existence of nonimmunological granulomas (perhaps better called "nonadaptive granulomas"), innate immunity mechanisms can suffice to drive granulomatous responses.…”
