Mechanical Bowel Preparation or Not? Outcome of a Multicenter, Randomized Trial in Elective Open Colon Surgery

Abstract: In the present study we could not detect a difference in outcome parameters between patients receiving mechanical bowel preparation in elective open colon surgery and patients without preoperative treatment of the bowel. The present study, although underpowered, did not show a difference in the primary outcome of bacterial wound cultures between patients receiving preoperative mechanical bowel preparation and patients receiving no preoperative bowel treatment. We conclude that there may be no need to continue … Show more

“…RCTs that stratified the risk of leakage to the site of anastomosis did not find any significant advantage of MBP for surgical procedures with rectal location of the disease. [1][2][3] As stated in our article, 4 we are confident that current literature provides strong evidence that in elective colorectal surgery, no significant benefit is derived from MBP. This applies to all procedures regardless of the site of anastomosis.…”

“…However, mechanical bowel preparation liquefies solid faeces, which could increase the risk of intraoperative spillage of contaminant 6,7 . Although some investigators believe that mechanical bowel preparation can reduce the bacterial load in the bowel, the large number of microorganisms in the digestive tract makes this almost impossible 8,9 . Mechanical bowel preparation has been shown to have Among non-surgical post-operative complications UTI was high in group A followed by respiratory and cardiac ones.…”

“…The first update included four new trials 17,21,29,30 and total 830 patients: 414 in Group A and 416 in Group B. Heterogeneous clinical outcomes were observed, but the authors suggested to skip the mechanical bowel preparation, as it would be associated with higher incidence anastomotic dehiscence in colorectal surgery.…”
