Mechanical alternans and restitution in failing SHHF rat left ventricles

Abstract: We examined mechanical alternans and electromechanical restitution in normal and failing rat hearts. Alternans occurred at 5 Hz in failing versus 9 Hz in control hearts and was reversed by 300 nM isoproterenol, 6 mM extracellular Ca2+, 300 nM −BAY K 8644, or 50 nM ryanodine. Restitution curves comprised phase I, which was completed before relaxation of the steady-state beat, and phase II, which occurred later. Phase I action potential area and developed pressure ratios were significantly reduced in the failing… Show more

“…condition can be provoked in normal isolated rat hearts by pacing with at least twice the normal heart rate, and it usually occurs after an abrupt jump in pacing rate [30]. The present pacing experiments do by far not reach this level.…”

“…The present pacing experiments do by far not reach this level. Alternating pulse is characterised by a substantial difference ∆P max LV changing its sign from beat to beat, and is related to insufficient recovery time for sarcoplasmatic Ca 2+ reuptake [30]. This seems to discriminate alternans from duorhythm.…”

“…SHHF rats of either sex were studied at 2 and 8 mo of age and after the development of overt heart failure. As previously described, heart failure was identified between 16 and 22 mo of age and was defined as a left ventricular ejection fraction Ͻ40% by echocardiography, with signs of labored breathing, cyanosis, subcutaneous edema, piloerection, cold extremities (tails), and orthopnea (8,26). Age-matched Wistar-Furth or Wistar rats (Harlan, Indianapolis, IN) of either sex were used as controls (8,31).…”

“…Picht et al (2006) used direct measurements of intra-SR free calcium to show that pacing induced calcium alternans does not require SR calcium content alternans but, rather, may depend more on RyR recovery. (Dumitrescu et al, 2002) In addition to artificial means of depressing SR calcium release, ischemia (Kurz et al, 1993;Mohabir et al, 1991;Wilson et al, 2006) and metabolic inhibition (Hüser et al, 2000;Kockskamper et al, 2002) have been associated with calcium transient alternans as well. In a series of studies, Qian et al (2003) have shown the development of significant alternans during the acute phases of ischemia.…”
