Objectives: To translate the Glasgow Children's Benefit Inventory (GCBI) questionnaire into Arabic language and assess its validity and reliability in scoring the benefit of patients after cochlear implantation in children. Methods: A cross-sectional study for a group of consecutive pediatric patients who underwent cochlear implantation between November 2018 and February 2020. The GCBI original questionnaire was translated into Arabic language and translated back to English by 2 different experts. The patients/parents were asked to complete the questionnaire given to them. Results: Seventy children were included in the study. The age at implantation ranged from 7 months to 13 years with a mean of 3.4 years (SD=2.3 years). The mean GCBI score was 52.2 (SD=24.0) ranging from-6.25 to 100.00. The internal consistency of the questionnaire was high (Cronbach's α=0.9). The 4-factor dimensions explained 55.1% of the variance. Conclusion: The GCBI questionnaire (Arabic version) is reliable tool to evaluate retrospectively the quality of life after an intervention in pediatric age for Arabic speaking population.

Objective: Patient-reported outcomes can be useful for reporting benefit from non-life-saving interventions, but often they report a single overall score, which means that much information on the specific areas of benefit is lost. Our aim was to perform a new factor analysis on the Glasgow Children’s Benefit Inventory (GCBI) to create subscales reflecting domains of benefit. Further aims were to assess the internal consistency of the GCBI, and to develop guidelines for reporting both a total score and sub-scales in future studies. Methods: We collected 4 existing datasets of GCBI data from children who have undergone tonsillectomy, ventilation tube insertion, pinnaplasty, and submucous diathermy to the inferior turbinates. We performed exploratory factor analysis with principal axis factoring with varimax rotation, we sought redundancy in question items, and we measured internal consistency. Results: Using the combined dataset of 772 cases, we found 4 factors which accounted for 64% of the variance and which we have labeled “Psycho-social,” “Physical health,” “Behavior,” and “Vitality.” Subscale results varied in predictable ways depending on the nature of the intervention. Cronbach’s alpha was 0.928. Item-total correlations were high, and no item could be deleted to improve alpha. Floor effects were apparent for various questions but were not consistent between different interventions. Conclusions: The GCBI contains a range of questions which each add value in different clinical interventions. We can now make recommendations for reporting the results of the GCBI and its 4 new subscales.

Cholesteatomas are bone destructive expansions of keratinizing squamous epithelium in the middle ear and temporal bone. Today, surgery is the only treatment. There are several controversies regarding cholesteatomas, including the definition, the pathogenesis and the surgical method. Intense efforts have been made searching for a comprehension of the cholesteatoma process at a cellular and molecular level. Recurrent infections and inflammation seem to be contributing factors for the cholesteatomas to expand. The innate immunity, essential to keep a healthy middle ear environment and to protect the middle ear from intruding pathogens, is therefore a matter of interest. In this thesis, results are presented from a cohort of cholesteatoma surgeries in Östergötland from a 16-year period. A group of patients also filled in a questionnaire to assess changes in health-related quality of life (HRQoL) after surgery. According to the findings in this thesis, the residual and recurrence frequencies are low, and the hearing and HRQoL are improved in the majority of cases. This thesis also presents an investigation of the innate immunity in ears with acquired cholesteatoma, in comparison to controls with healthy middle ears. The expression of mRNA of toll like receptors 2 and 4, participants of the Janus kinase/signal transducer and activator of transcription pathway, and nitric oxide synthases in middle ear mucosa, were investigated with quantitative polymerase chain reaction. An investigation of nitric oxide (NO) in the middle ear, with chemiluminescence measurements, is also presented. A derangement of the innate immune system is seen in ears with cholesteatoma, which supports the idea that the innate immunity participates in the cholesteatoma process, though the underlying mechanisms are still unclear. The suggestion of NO production in the middle ear sheds light on NOs possible participation in the healthy middle ear environment.

Abstract: The Portuguese GBI and GCBI proved to be valid and reliable to evaluate HRQoL following otoplasty in Portuguese-speaking children. The outcome was very favourable; almost all patients would recommend the surgery.
