Mentioned for the first time in 2005, the exposome concept initially emerged in response to the need to reflect in research the significant impact of the environment on health and disease. Since its emergence, in line with the rapid developments in general knowledge and technology, as well as with the dramatic changes in geoclimatic conditions, the notion has been in remarkable progress. Originally meant as a call for the development of more complex methods of environmental exposure assessment, applicable at individual level and throughout the individual's life span, the scope of the concept has now been expanded to also incorporate the associated biological response. In result of over 15 years of research and evolution, exposomics now involves adopting a global and multidisciplinary approach meant to integrate and develop the top-down, bottom-up and functional study strategies already established in the field, as well as their complementation with other methods, increasing efforts for their standardization and validation, thus rendering exposomics increased utility in clinical practice. This review is focused on the introduction of the exposome concept and the development of research from its perspective that can only be achieved under the conditions of multidisciplinary collaboration, with the participation of scientific fields such as environment, social sciences, analytical chemistry, molecular biology, toxicology, epidemiology and medicine. RezumatMenționat pentru prima dată în 2005, conceptul de expozom a apărut inițial ca răspuns la nevoia de a reflecta asupra impactului semnificativ al mediului asupra sănătății și bolii. De la apariția sa, în concordanță cu evoluțiile rapide ale cunoștințelor generale și ale tehnologiei, precum și cu schimbările dramatice ale condițiilor geoclimatice, noțiunea a cunoscut un progres remarcabil. Înțeles inițial ca un apel pentru dezvoltarea unor metode mai complexe de evaluare a expunerii la mediu, aplicabile la nivel individual și pe toată durata de viață a individului, domeniul de aplicare a conceptului a fost acum extins pentru a include și răspunsul biologic asociat. După 15 ani de cercetare și evoluție, expozomica presupune acum adoptarea unei abordări globale și multidisciplinare menite să integreze și să dezvolte strategiile de studiu după metodele top-down, bottom-up și cele funcționale deja stabilite în domeniu, dar și să le completeze cu alte metode, sporind eforturile de standardizare și validare a acestora și dând astfel expozomicii o utilitate sporită în practica clinică. Această lucrare se concentrează pe introducerea conceptului de expozom și dezvoltarea cercetării din perspectiva acestui concept, care poate fi realizată numai în condițiile unei colaborări multidisciplinare, din mai multe domenii științifice precum mediul, științele sociale, chimia analitică, biologia moleculară, toxicologia, epidemiologia și medicina.

Objective We aimed to better understand the challenges related to type 2 diabetes medication-taking through Theoretical Domains Framework (TDF)-guided interviews with people with type 2 diabetes with varying degrees of medication-taking. Methods One-on-one qualitative interviews following a semistructured discussion guide informed by the TDF were conducted. Thirty people with type 2 diabetes in Canada were interviewed, with representation from across the country, of both sexes (47% female), of people with various diabetes durations (mean 12.9 ± 7.9 years), with different types of medication plans (n = 15 on polypharmacy), and with various medication-taking levels (n = 10 each for low-, medium-, and high-engagement groups). Results Themes related to medication-taking from interviews mapped to 12 of the 14 TDF theme domains, with the exclusion of the knowledge and skills domains. The most prominent domains, as determined by high-frequency themes or themes for which people with low and high medication-taking had contrasting perspectives were 1) emotion, 2) memory, attention, and decision processes, 3) behavioral regulation, 4) beliefs about consequences, 5) goals, and 6) environmental context and resources. Conclusion Through our interviews, several areas of focus emerged that may help efforts to increase medication-taking. To validate these findings, future quantitative research is warranted to help support people with type 2 diabetes in overcoming psychological and behavioral barriers to medication-taking.
