Measurement of TSH receptor blocking immunoglobulins using 3H‐adenine incorporation into FRTL‐5 and JPO9 cells: use in a child with neonatal hypothyroidism

Abstract: This case of neonatal hypothyroidism associated with the passage of thyroid blocking antibodies demonstrates the utility of this new assay in the differential diagnosis of neonatal hypothyroidism.

“…2a), this serum dilution seems to be optimal for the determination of TSAB in hypotonic Krebs Ringer Buffer. These findings are in agreement with the serum buffer dilution of 1/5–1/10 in hypotonic Hanks buffer used by Michelangeli et al (1994, 1995). Considering a normal IgG concentration of 10–18 g/l and the used serum/buffer dilution of 1/10 our TSHR AB bioassay was performed with an IgG concentration of 1–1.8 g/l.…”

“…DELFIA free thyroxine (FT4) and DELFIA hTSH kits were purchased from Kabi Pharmacia (Wallac Oy, Turku, Finland). The interassay coefficients of variation were 11% (6-8 pmol/L), 7% (16)(17)(18)(19) pmol/L), and 8% (43-53 pmol/L) for the fT4 assay, and 6% (0.77-1.03 mU/L), 4% (4.3-5.7 mU/L), and 3% (22)(23)(24)(25)(26)(27)(28)(29)(30) (20,21). For as¬ says, the cells were subcultured onto 12-well plates and grown to confluence.…”

“…We have recently reported new assays for the measurement of TSAb and TBAb using a method involving incorporation of [3H]adenine into FRTL-5 or JP09 cells and measuring the intracellular production of cAMP directly (17)(18)(19). These assays can be used to measure the specific activities of antibodies in serum.…”

“…Total T 3 and T 4 concentrations were determined using a corresponding EIA kit from Diagnostic Systems Laboratories (Webster, TX, USA). The activity of TSH receptor (TSHR)-stimulating antibody (TSAb) was measured by cell bioassay as described by Costagliola et al (1992) and Michelangeli et al (1995), and expressed as a percent increase in TSHRinduced cAMP production compared to the normal pool serum. Antithyroid peroxidase antibodies (anti-TPO Ab) were detected using a radioimmunoassay DYNO test antiTPOAb (Brahms Diagnostica GmBH, Berlin, Germany), with a cut-off for positivity of 100 U/ml (Feldt-Rasmussen et al 1991).…”
