Background: Gestational diabetes is diabetes, or high blood sugar levels, that develops during pregnancy. It occurs in about 4% of all pregnancies. It is usually diagnosed in the later stages of pregnancy and often occurs in women who have no prior history of diabetes. Aim of the Work: To assess the predictive value of elevated glycosylated hemoglobin at 34 weeks' gestation with adverse fetal outcome as regard fetal macrosomia and neonatal hypoglycemia. Patients and Methods: This prospective longitudinal cohort study included 98 pregnant women who were recruited from the obstetric outpatient clinic and department at Al-Galaa Teaching Hospital. Results: HbA1c ≥7.9 has sensitivity of 88.1% and specificity of 66.1%, in prediction of macrosomia and a sensitivity of 91.9% and specificity of 63.9% in prediction of Hypoglycemia. Conclusion: HbA1c ≥7.9 has moderate diagnostic characteristics in prediction of macrosomia, and hypoglycemia, low diagnostic characteristics in prediction of RDS and NICU. Recommendations: Use of HbA1C is recommended for patients with GDM for screening, follow up and prediction of adverse neonatal outcomes.

Đặt vấn đề: Phương pháp sắc ký lỏng hiệu năng cao trao đổi ion (ion exchange High Pressure Liquid Chromatography-HPLC) và điện di mao quản (Capillary Electrophoresis-CE) có kết quả tương hợp tốt, không có sự khác biệt giữa hai phương pháp khi phân tích trên mẫu máu hemoglobin bình thường. Nghiên cứu được thực hiện nhằm so sánh kết quả định lượng HbA1c giữa hai phương pháp trên các mẫu bệnh hemoglobin. Mục tiêu nghiên cứu: So sánh độ chênh và độ tương hợp về kết quả giữa phương pháp sắc ký lỏng hiệu năng cao trao đổi ion với điện di mao quản trong định lượng HbA1c trên mẫu máu bệnh hemoglobin. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 78 mẫu máu có kết quả bất thường điện di hemoglobin tại Bệnh viện Huyết học-Truyền máu Cần Thơ và Bệnh viện Trường Đại học Y Dược Cần Thơ từ tháng 4/2021 đến tháng 2/2022. Kết quả: Tuổi trung vị trên mẫu máu nghiên cứu là 20 (tuổi nhỏ nhất là 01 và lớn nhất là 70); giới tính nam chiếm tỷ lệ 43,6% và nữ chiếm 56,4%. Trung bình RBC là 5,12±1,11x1012/L, HGB là 110±27,6g/L, MCV là 70,9±9,39fL, MCH là 21,7±3,93pg. Số mẫu có biến thể HbE là 23, HbH là 13 mẫu và β-thalassemia là 42 mẫu. Độ chênh lệch kết quả xét nghiệm HbA1c giữa hai phương pháp HPLC và CE trên tổng số mẫu nghiên cứu là -0,83±1,19(%), trong nhóm có HbE là -1,84±1,43(%), nhóm HbH là -0,89±0,82(%) và nhóm β-thalassemia là 0,26±0,69(%); hai phương pháp không tương hợp về kết quả định lượng HbA1c trong tất cả các nhóm. Kết luận: Kết quả định lượng HbA1c giữa phương pháp sắc ký lỏng hiệu năng cao trao đổi ion với điện di mao quản có độ chênh lệch trung bình là -0,83±1,19(%); kết quả trung bình HbA1c bằng phương pháp CE cao hơn phương pháp HPLC, sự khác biệt có ý nghĩa thống kê (p<0,05) và không tương hợp trên mẫu máu bệnh hemoglobin.
