Although conscience, as an ethical concept, has emerged widely in the field of nursing, its functional meaning and its effects on nurses' performance are not clear. Therefore, the present study aimed to analyze the concept of conscience in the context of Iranian nurses’ ethical performance. This study used a hybrid model including theoretical, fieldwork, and final analytic stages. In the theoretical phase, English and Persian articles published up to 2020 and indexed by scientific databases were analyzed. In the fieldwork phase, semi-structured in-depth interviews were conducted on nurse participants. The last two stages were jointly considered to draw the study’s conclusions. In the theoretical phase, conscience was considered as a context-dependent concept, an inner voice, and a criterion for distinguishing right from wrong. The fieldwork phase’ results were categorized into three themes: “perception of conscience”, “commands of conscience”, and “obedience to conscience. The final definition was reached by merging the theoretical and field stages. This article aimed at investigating the relevance of conscience to ethical practice in the nursing field. Findings show that conscience is an inner feeling or voice that plays a vital role in providing ethical care by nurses.

Aims To describe lived experiences of spirituality from the perspective of people who have been subject to inpatient psychiatric care and to interpret these experiences from an understanding of health as dialectical. Methods After approval from a regional ethical board, eleven participants were recruited from two organisations for people with mental health problems. Participants were asked to narrate about spiritual experiences and occasions where such experiences had come close. The transcribed interviews were analysed by means of a phenomenological hermeneutical approach. Findings A structural analysis of the text resulted in three themes; perceiving the presence of something extra mundane, making sense of reality and struggling for acceptance. The comprehensive understanding highlights spiritual experiences as going beyond religion, even though religious experiences appear as part of it. These experiences can indeed be a resource contributing to experiences of hope, connectedness, meaning and coherence in life. However, they can also give rise to doubt, anxiety and feelings of loneliness and hopelessness. Rather than understanding spiritual experiences as being either ‘good’ or ‘bad’, we could approach spirituality as something that is always present in alternate and inter‐related forms. Metaphorically, this could be understood as a ‘near‐life experience’, summarising participants’ experiences related to their struggle with issues related to suffering and health which are simultaneously present. Conclusions If psychiatric nurses could approach this complexity and, without being judgemental, explore seemingly positive and negative experiences of spirituality as dialectically related to each other, rather than viewing them as either resources or problems, this could contribute to insiderness care and hopefully also support people who struggle with these experiences to seek help when needed.

Öz Amaç: Zihinsel bir süreç olan vicdan, özel ve mesleki yaşamda tutum ve davranışlara etki eden, felsefi, etik ve ahlaki yönü bulunan çok boyutlu bir kavramdır. Hemşirelerin çalışma hayatlarında hissettikleri vicdani rahatsızlık, ilerleyen süreçte suçluluk, hüzün, umutsuzluk ve güçsüzlük gibi olumsuz duygular yaşamalarına ve mesleki tükenmişliklerine sebep olabilir. Yöntem: Bu araştırma metodolojik bir araştırma olup, 163 hemşire ile yapılmıştır. Verilerin toplanmasında tanıtıcı bilgi formu ve Dahlqvist ve arkadaşları tarafından geliştirilen vicdan algısı ölçeği kullanılmıştır. Ölçeğin dil uyarlaması ve içerik geçerlilik incelemesinin değerlendirilmesinin ardından veriler, kapsam geçerlilik için uzmanlar arası uyum bakımından değerlendirilmiştir. Yapı geçerliliği için ise faktör analizi yapılmıştır. Güvenilirlik çalışması, madde toplam korelasyonu, Cronbach alfa katsayıları ve test tekrar test ile incelenmiştir. Bulgular: Ölçeğin kapsam geçerliliği değerlendirme puanları bakımından uzman görüşleri arasında uyum saptanmıştır (Kendall's W=0,114; p=0,101 > 0,05). Ölçeğin madde-toplam puan korelasyon katsayıları (r) 0,090 -0,697 arasında olduğu ve aralarındaki ilişkinin pozitif yönde istatistiksel yönden ileri düzeyde anlamlı olduğu (p < 0,001) bulunmuştur. Güvenirlik analizlerinde Cronbach Alpha katsayısı 0,84 ve test tekrar test korelasyon katsayısı r=0,97'dir. Sonuç: İstatistiksel değerlendirmede, 16 maddeli ölçeğin 3 maddesinin ölçekten çıkartılarak, 13 maddeli haliyle Türkiye'deki hemşirelerle ilgili yapılacak çalışmalarda kullanılması uygun bulunmuştur. AbstractObjective: Conscience is a mental process that affects attitudes and behaviors in private and professional life and a multidimensional concept with philosophical, ethical and moral aspects. The conscientious disturbance experienced by nurses in professional life may lead to negative feelings such as guilt, sadness, hopelessness and weakness, and may cause them to experience professional burnout in the future. Method: This study is a methodological research, and the study consisted of 163 nurses. Data were collected using a personal information form and the Perceptions of Conscience Questionnaire developed by Dahlqvist et al. After the assessment of the language adaptation and content validity of the scale, data were analyzed in terms of inter-rater agreement for content validity. Factor analysis was used for construct validity. The reliability of the instrument was evaluated using item total correlation, Cronbach's alpha coefficients and test-retest analyzes. Results: There was an inter-rater agreement in terms of the content validity evaluation scores of the questionnaire (Kendall's W=0.114; p=0.101 > 0.05). The item-total score correlation coefficients (r) of the scale were between 0.090 and 0.697, and there was a positively significant relationship between them (p<0.001). Meanings of troubled conscience and how to deal with it: expressions of Persian‐speaking enrolled nurses in Sweden

Abstract: The enrolled nurses understood themselves as caring people and not only caregivers. They knew that they should hear their conscience and respond to it by trying to be a caring person and acting according to their values. The findings should be interpreted in the given specific context.
