Mean platelet volume: comparison of three analysers towards standardization of platelet morphological phenotype

Abstract: To harmonize our procedures, we propose definitions of platelet size (normal size, macroplatelets and giant platelets) based on the coordinated interpretation of the MPV, the distribution of platelet volume and the morphological appearance.

“…5 Moreover, instruments that operate on different principles induce variability into the measurement of platelet volume in patients with nonmacrocytic thrombocytopenias as well as in healthy participants, thus making it difficult to compare values obtained in different centers. 6 As a consequence, what we presently know about this subject is derived mainly from case reports, series of patients with one form of IT, and two small studies that directly compared platelet size in some of the known ITs. 5,7 To advance knowledge in this field (taking into account the present limitations of cell counters), we performed a collaborative study to measure platelet size by image analysis of peripheral blood smears obtained from patients affected by all 19 forms of IT currently known.…”

“…In many cases of thrombocytopenia, large platelet are seen in peripheral smear, this size of platelet was suggested to help in deciding the category of thrombocytopenia long back (3,4) . Initially size was noted by microscopic studies only (5) . Now automated analyzers give objective assessment of platelet volumes.…”

“…Importantly, some of these new parameters have demonstrated useful clinical applications. [10][11][12][13][14][15][16][17][18] For example, immature reticulocyte fraction, which is defined as the ratio of immature to total reticulocytes, has been proposed as a new hematologic parameter in the evaluation of erythropoietic activity. 10 Its main clinical value is that the immature reticulocyte fraction has been shown to be an earlier and more sensitive indicator of bone marrow stimulation than other traditionally used reticulocyte parameters, such as the absolute reticulocyte count and the reticulocyte percentage.…”
