Objective: Obstructive sleep apnea (OSA) syndrome is a common disorder that can cause hypercoagulation. Mean platelet volume (MPV) and platelet distribution width (PDW) are associated with hypercoagulability. This study aimed to investigate whether MPV and PDW values change in patients with OSA who were treated with continuous positive airway pressure (CPAP) device.Methods: A total of 43 adult patients with OSA who were treated with CPAP were included in this retrospective study. Patients who underwent CPAP treatment for <5 days/week and <4 h/day were excluded. Blood parameters, including MPV and PDW, were recorded before CPAP treatment and at the third month of CPAP treatment. All patients underwent polysomnographic evaluation with full night polysomnography and in the second night CPAP titration was performed together with full night polysomnography and MPV and PDW values were statistically compared before and after CPAP treatment.Results: Apnea hypopnea index was significantly reduced, whereas oxygen saturation was significantly increased at CPAP titration night (p<0.001). The third month of CPAP treatment resulted in significantly low PDW (p=0.004) values, but MPV values did not change. Conclusion:PDW value at the third month of CPAP treatment revealed a significant improvement compared with the values before treatment; moreover, no change was observed in MPV values. It was concluded that in patients with OSA who were treated with CPAP, activation of platelets may result in recovery.

BackgroundObstructive sleep apnea (OSA) is a novel risk factor for acute coronary syndrome (ACS). Several studies have shown OSA to be associated with induced platelet reactivity. However, whether OSA have effects on platelet function profiles in ACS patients taking dual antiplatelet therapy remains unexplored.Methods and ResultsThis was a cross‐sectional observational study, in which ACS patients taking maintenance aspirin and clopidogrel therapy were included. OSA was defined as an apnea‐hypopnea index ≥15 events/hour. The inhibitory rate of arachidonic acid or adenosine diphosphate pathway were assessed with thrombelastography and defined patients with high residual on‐treatment platelet reactivity. Platelet indices were obtained from routine analysis of blood samples using an automated blood cell counter. A total of 127 ACS patients taking dual antiplatelet therapy were analyzed. Platelet volume indices, including mean platelet volume and platelet large cell ratio, were significantly increased in patients with OSA. Patients with OSA (n=68) had significantly lower inhibitory rate of adenosine diphosphate receptor pathway (P=0.028) compared with those without (n=59). After adjustment for potential confounders, patients with OSA were more likely to have high residual on‐treatment platelet reactivity after clopidogrel therapy (adjusted odds ratio: 3.25, 95% confidence interval: 1.19–8.87, P=0.021).ConclusionsIn ACS patients taking dual antiplatelet therapy, OSA is associated with an increased level of platelet volume indices, reduced clopidogrel‐induced antiplatelet effects and a greater prevalence of high residual on‐treatment platelet reactivity.

SUMMARYPlatelet indices in obstructive sleep apnea: the role of mean platelet volume, platelet distribution widht and plateletcrit Introduction: Because studies report controversial findings of mean platelet volume (MPV) and platelet distribution width (PDW), we aimed to investigate platelet indices in patients with obstructive sleep apnea (OSA). Materials and Methods:One hundred seventy three patients with OSA and 43 control subjects included in analyses. Patients with an apnea-hypopnea index (AHI) ≥ 5 were considered to have OSA and an AHI < 5 were considered as control. Platelet indices (MPV, PDW, plateletcrit) were obtained from routine analysis of blood samples using an automated blood cell counter. OSA, especially in severe OSA when compared to control group (16.80 ± 0.56 vs. 16.21 ± 1.51, p= 0.015 and 16.82 ± 0.06 vs.16.21 ± 1.51, p= 0.014, respectively). Correlation analysis showed a significant but a weak correlation between the PDW and AHI (r= 0.161, p= 0.018) and the PDW and the desaturation index (r= 0.171, p= 0.016). The ROC curve analysis showed that the optimal cut-off value, distinguishing patients with OSA from those in control group, for PDW was 16.62 (sensitivity 57.8%, specificity 58.1%, AUROC 0.60, p= 0.042) Results: Among platelet indices, only PDW was higher in patients with . After the exclusion of patients with comorbid diseases (cardiovascular diseases, diabetes mellitus and chronic inflammatory diseases), we also found weak but a significant correlation between PDW and AHI (r= 0.179, p= 0.040).
