The odontogenic keratocyst (OKC) well-known for its aggressiveness and high recurrence rate, comprises approximately 11% of all jaw cysts. Due to its aggressive behavior it was placed into category of tumour in 2005 by the World Health Organization (WHO). Objectives: The purpose of this study was to determine the Ki-67 expression in Odontogenic Keratocysts to predict its proliferative potential. Study Design: Descriptive study. Setting: Department of Morbid Anatomy and Histopathology, UHS. Periods: June 2014- June 2018. Material & Methods: This is a descriptive study comprising of 39 cases of odontogenic cysts. These surgically removed samples were processed at University of Health Sciences (UHS) laboratory. Routine staining with Hematoxylin & Eosin stain along with immunohistochemistry (IHC) with Ki-67 antibody was performed. Immunohisto chemical scoring was done on the basis of percentage of the nuclear staining of Ki-67. Data was entered into SPSS 22 and descriptive statistics were measured in the form of percentage and frequency. Quantitative variables such as age of patient, size of the cyst, and Ki-67 score were also measured. P value <0.05 was taken as significant. Results: The mean age of the patients was 25.08 ±14.5 years. Significant association was observed between histological variables with odontogenic keratocyst such as parakeratinized epithelial lining (p = 0.00), epithelial hyperplasia both typical and atypical (p = 0.02) and focal spongiosis (p = 0.04). Foci having epithelial atypia demonstrated stronger staining intensity compared to adjacent normal epithelium. However, no significant association was observed between the histological variables and Ki-67 expression. Conclusion: OKC expressed low Ki-67 expression in most of the cases, however, foci of strong expression were also observed in few cases.

Ki-67 is a marker of cell proliferation, used as an important diagnostic marker in the pathologic differentiation of various lesions. It is also relevant for developing targeted molecular therapies. We carried out a systematic review to assess the Ki-67 labeling index (LI) in odontogenic cysts and tumors. Databases were searched, including PubMed (MEDLINE), Scopus, CINHAL, PsycoInfo, the Cochrane Library, and Proquest. The meta-analysis was carried out based on the data of 608 lesions. When a 5% cut-off point was set, ki-67 LI of all benign odontogenic tumors dropped below this point. All the malignant tumors demonstrated an LI of over 15.3%; a significantly higher Ki-67 LI in malignant odontogenic lesions (17.59±2.80) was observed. Among benign tumors, the largest and the smallest Ki-67 LIs were seen in ameloblastoma (4.39±0.47) and adenomatoid odontogenic tumor (0.91±1.71). The mean values of Ki-67 LI in tumors and cysts were 4.23 (0.38) and 1.04 (0.07), respectively. Among odontogenic cysts, the highest Ki-67 LI was found in odontogenic keratocyst (OKC) (3.58±0.51), and the lowest in the radicular cyst (1.29±0.62%). Ki-67 LIs in all odontogenic cysts were <3%, except for OKC. This controversial lesion seems to have a profile more similar to a tumor, and a treatment plan similar to tumors might be suggested. We found that odontogenic lesions have diverse proliferative activities that help differentiate between various lesions and suggest therapeutic plans.

Background Odontogenic keratocyst (OKC) is a development cyst, of odontogenic origin, that differs from other entities by its infiltrating and aggressive biological behavior. Among conservative treatments for large lesions, surgical decompression stands out, with a variable recurrence rate. Aim: To evaluate the histological effects of decompression treatment on OKC, including cell proliferation and apoptosis of epithelial cyst. Material and Methods 21 OKC cases were included. Samples were taken before and after surgical decompression for histological evaluation and immunohistochemical staining of Ki-67, MCM4/7, Bax and Bcl2. Data were analyzed and compared using Student’s t and Wilcoxon tests for related samples, and p values <0,05 were considered statistically significant. Results After decompression treatment an increase in inflammation of the cystic wall ( p =0,029), loss of parakeratinization of the epithelium ( p =0,007) and absence of palisade cell distribution in the basal layer were observed ( p =0,002). There were no statistically significant changes in the expression of Ki-67 ( p =0,323), MCM4/7 ( p =0,079), Bax ( p =0,392) or Bcl-2 when compared before and after decompression. Conclusions Surgical decompression generates histological structural changes in OKC both in the epithelium and connective wall, however, these findings do not seem to alter induction of the cell cycle or epithelial apoptosis.
