Maximum Standardized Uptake Value of Fluorodeoxyglucose Positron Emission Tomography/Computed Tomography Is a Prognostic Factor in Ovarian Clear Cell Adenocarcinoma

Abstract: Maximum SUV on preoperative FDG-PET/CT in ovarian epithelial cancer differs according to histologic type. In clear cell AC, SUVmax may represent a prognostic factor.

“…The SUV max has been suggested as a significant parameter for predicting clinical outcomes in EOC. However, SUV max is just a point measurement of highest metabolic activity and provides no additional information concerning tumor burden . Recently, several reports have suggested that volumetric parameters of 18 F‐FDG PET/CT, that is, MTV and TLG, comprehensively reflect both metabolic activity and tumor burden in a 3‐D way, and predict survival more accurately than SUV max in EOC .…”

“…The lack of prognostic value for the semiquantitative parameters may derive from the heterogeneous histological subtypes of the malignancy, as SUV max may differ by histological subtype [6]. However, the contention may also relate to the patient sample.…”

“…1 8 F-Fluoro-2-deoxyglucose positron emission tomography/computed tomography ( 18 F-FDG PET/CT) has been investigated in patients with epithelial ovarian cancer, displaying high diagnostic accuracy, with significant correlation to histopathology [4,5]. Its application during the staging process plays a pivotal role in the prognostic assessment, as reported by many studies correlating qualitative and semiquantitative parameters to different outcomes [6][7][8][9][10]. In addition, volumetric uptake parameters such as metabolic tumour volume (MTV) and total lesion glycolysis (TLG) have been proven to be reliable prognostic factors [11,12].…”

“…In recent years, FDG-PET/CT has been utilized not only for its production of morphological images, but also for its ability to provide information regarding biological functions and metabolism. The maximum standardized uptake value max (SUVmax) of FDG-PET/CT that reflects glucose metabolism of tumors has been reported to be an imaging biomarker for prognostic prediction, therapy evaluation, and chemosensitivity [5]- [8]. We previously reported that the SUVmax of pretreatment primary tumors differs depending on the histological type of epithelial ovarian cancer, that SUVmax is lower in CCC than in serous carcinoma and endometrioid carcinoma, and that SUVmax can be a biomarker to predict prognosis in CCC [5].…”

When the SUVmax reflects the biological characteristics of the disease, even at stage I CCC, it can be a useful criterion for treatment decisions.…”
